What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Ic - Items Taken To Station Disposed/Dedicated Or Disinfected

    The inspectors’ words, 351 words

“IC-IF TO STATION=DISP/DEDICATE OR DISINFECT CFR(s): 494.30(a)(1)(i) Items taken into the dialysis station should either be disposed of, dedicated for use only on a single patient, or cleaned and disinfected before being taken to a common clean area or used on another patient. -- Nondisposable items that cannot be cleaned and disinfected (e.g., adhesive tape, cloth covered blood pressure cuffs) should be dedicated for use only on a single patient. -- Unused medications (including multiple dose vials containing diluents) or supplies (syringes, alcohol swabs, etc.) taken to the patient’s station should be used only for that patient and should not be returned to a common clean area or used on other patients. This Standard is not met as evidenced by: 29732 Based on observation, interview and policy review, the facility failed to ensure all equipment used on patients are disinfected after each use. This could potentially affect all patients and staff. The census was 51. 1. During an observation on 10/08/24 at 7:14 AM, Staff C used a stethoscope on the patient at station 13. At 7:18 AM, Staff C used the stethoscope on the patient at station 2 without cleaning it between patients. During an interview on 10/08/24 at 7:28 AM, Staff C confirmed not cleaning the stethoscope between patients. 2. During an observation on 10/08/24 at 10:21 PM, Staff C and E transferred Patient #1 from the dialysis chair to a wheelchair using a mechanical lift. Staff E returned the mechanical lift to the designated clean area and plugged it in, then walked away. Staff E did not clean the Hoyer lift before returning to the clean area. At 11:36 AM the mechanical lift was used on Patient #7. On 10/08/24 at 12:05 PM the Clinical Managers Staff B and K were made aware of this finding. Review of the facility policy titled “Guidelines for Use of the Hoyer Lift”, effective 08/30/24, revealed the sling must be surface disinfected with l :100 bleach solution after use. Review of the facility policy titled “Cleaning and Disinfection General Policy for Inpatient Services” effective 11/06/23 revealed disinfect the stethoscope after each use.”
