What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“PA-APPROPRIATENESS OF DIALYSIS RX CFR(s): 494.80(a)(2) The patient’s comprehensive assessment must include, but is not limited to, the following: (2) Evaluation of the appropriateness of the dialysis prescription, This Standard is not met as evidenced by: 29000 Based on observation, medical record review, staff interview, and review of the facility policy, the facility failed to follow their policy and physician orders for machine settings for one (Patient #1) out of seven patients observed for machine settings. The facility census was 33. Findings include: Review of the medical record for Patient #1 revealed physician orders for treatment which included sodium bicarbonate (HCO3) of 40. Observations on 09/15/25 at 10:00 AM with Staff E and Staff F revealed Patient #1 was receiving hemodialysis at Station #8. The machine settings revealed the HCO3 was set at 30 not the prescribed order of 40. Staff E increased the HCO3 to 35 immediately stating “Need to increase this gradually for the patient.” Review of the treatment run sheet for 09/15/25 listed the Bicarb setting of 40 and was signed off by Registered Nurse (RN) Staff F and a Patient Care Technician (PCT) Staff D. Review of the policy titled “Pre-Treatment Safety Checks Procedure,” dated 02/06/23, revealed staff should verify the following elements by two direct patient care staff members prior to treatment initiation which include prescribed dialysate composition and proper bicarbonate has been entered per prescription. Interview on on 09/15/25 at 10:40 AM Staff E verified the findings of not following the prescribed bicarbonate during treatment for Patient #1.”
