What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 367 words

“PQ-STAFF LIC AS REQ/QUAL/DEMO COMPETENCY CFR(s): 494.140 All dialysis facility staff must meet the applicable scope of practice board and licensure requirements in effect in the State in which they are employed. The dialysis facility’s staff (employee or contractor) must meet the personnel qualifications and demonstrated competencies necessary to serve collectively the comprehensive needs of the patients. Findings: Personnel file for Staff I (Registered Nurse/RN) revealed Staff I had a hire date of 9/25/2023. Staff I completed orientation and training on 12/11/2023. Staff I’s Procedural Skills Verification Checklist was not completed until 12/23/2023 and 12/26/2023. The Patient Assignment Schedule, dated from 12/12/2023 - 12/23/2023, revealed Staff I worked independent patient assignments prior to 12/26/2023. Personnel file for Staff M (Patient Care Technician/PCT) revealed Staff M had a hire date of 10/2/2023. Staff M completed orientation and training on 12/13/2023. Staff M’s Procedural Skills Verification Checklist was not completed until 12/24/2023. The Patient Assignment Schedule, dated from 12/14/2023 - 12/24/2023, revealed Staff M worked independent patient assignments prior to 12/24/2023. The facility policy and procedure (P&P) titled, “Training Programs for New Patient Care Provider Teammates,” last dated 4/2022, stated, “Successful completion of the clinical component of the modality... includes documentation of proficiency as evidenced by the completion of the Procedural Skills Verification Checklist.” Per interview of Staff C (Group Facility Administrator) on 12/27/2023 at 11:00AM, Staff C acknowledged these findings.”
