What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Pe - Equipment Maintenance - Manufacturer’S Dfu

    The inspectors’ words, 404 words

“PE-EQUIPMENT MAINTENANCE-MANUFACTURER’S DFU CFR(s): 494.60(b) The dialysis facility must implement and maintain a program to ensure that all equipment (including emergency equipment, dialysis machines and equipment, and the water treatment system) are maintained and operated in accordance with the manufacturer’s recommendations. This Standard is not met as evidenced by: 28297 Based on observation, interview, and policy review, the facility failed to maintain seven station chairs (A1, A5, A10, B2, B6, B8, and B9) out of 20 stations in a safe, functional, and comfortable treatment environment. The failure to maintain station chairs had the potential for patient discomfort and the inability to clean the chairs in a sanitary manner had the potential to affect 106 in-center patients. Findings include: During an interview on 07/29/24 at 1:40 PM, Patient (P)11 complained about the chairs being old, uncomfortable and in disrepair. Observation on 07/29/24 at 1:40 PM revealed the chairs at Station B2 had tears in both armrests with the under foam exposed. The chair at Station B8 had a tear in the left armrest with the foam exposed. During an interview on 07/29/24 at 1:45 PM Patient Care Technician (PCT) 1 confirmed that many of the chairs were in disrepair and old. Observation on 07/29/24 at 3:45 PM revealed the chair at Station B9 had tears in both armrests with the under foam exposed. The chair at Station A1 had tears in the armrest in two different places with the under foam exposed. The chair at Station A5 chairside tables had multiple stains of brown and black substance that made it difficult to tell if the chair was properly cleaned. The chair at Station B6 had tears in both armrests with the under foam exposed. The chair at Station A10 had tears in both armrests with the under foam exposed. Observation on 07/31/24 at 11:55 AM revealed the chair at Station B9 with cracks and tears going across the entire seat cushion with the underlining and foam exposed. The entire seat cushion was in disrepair. During an interview on 07/31/24 at 12:00 PM, the Facility Administrator observed and confirmed the chair at Station B9 was in disrepair, and that there were other chairs that needed repairs. Review of the facility’s policy “Physical Environment,” revised April 2018 indicated the following: “ ...The dialysis facility will be designed, constructed, equipped, and maintained to provide dialysis patients, teammates, and the public a safe, functional, and comfortable treatment environment ...””
