What these words mean4
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Condition-level deficiency
For hospices, home health agencies, hospitals and dialysis facilities, CMS writes deficiencies at two levels. Standard is the usual one; condition-level is the more serious, when a problem is serious enough that the provider doesn’t meet a whole condition of taking part in Medicare. • Cfc - Water And Dialysate Quality

    The inspectors’ words, 206 words

“CFC-WATER & DIALYSATE QUALITY CFR(s): 494.40 This Condition is not met as evidenced by: 39223 Based on observation, review of facility documents, and interview with staff, it was determined that the facility failed to ensure that the door allowing access to the Water Treatment Room is secured (V-184). On 08/27/25 at 9:52 AM, an Immediate Jeopardy (IJ) was identified for the facility’s failure to ensure that the door allowing access to the Water Treatment Room was secured. On 08/27/25 at 12:57 PM, the IJ template was presented to the Facility Administrator (FA), and a Removal Plan was requested. On 08/27/25 at 4:07 PM, an acceptable Removal Plan was received. The facility implemented the following: observation that the door allowing access to the Water Treatment Room was fully closed and locked; interviews with staff revealed staff were educated and all verbalized that the door allowing access to the Water Treatment Room should be secured at all times; review of staff education and sign-in records; and review of Governing Body meeting minutes dated 8/27/25. The IJ was removed on 08/28/25 at 9:41 AM, after the State Survey Agency verified full implementation of the Removal Plan, and Condition Level non-compliance remains (V-184). Cross Reference: 494.40(a) - Environment-Secure & Restricted”

    Rule V0175. Condition-level. From a complaint.

  • Environment: Secure And Restricted

    The inspectors’ words, 347 words

“ENVIRONMENT-SECURE & RESTRICTED CFR(s): 494.40(a) 8 Environment: secure & restricted The water purification and storage system should be located in a secure area that is readily accessible to authorized users. The location should be chosen with a view to minimizing the length and complexity of the distribution system. Access to the purification system should be restricted to those individuals responsible for monitoring and maintenance of the system. This Standard is not met as evidenced by: 39223 Based on observation, review of facility documents, and staff interview, it was determined that the facility failed to ensure that the door allowing access to the Water Treatment Room is secured. Findings include: During a tour of the Hemodialysis Treatment Floor on 08/27/25 at 9:51 AM, the doors to the Storage Room, located off the Hemodialysis Treatment Floor, and the Water Treatment Room, located inside the Storage Room, were not able to fully close on their own due to a misalignment of the doors latching mechanism. This surveyor was able to access both the Storage Room and the Water Treatment Room unaccompanied by facility staff. The Storage Room and the Water Treatment Room doors did not restrict unauthorized persons from entry into those areas. During an interview at 11:12 AM, Staff (S)2 stated that he/she was in the Water Treatment Room in the morning and that when he/she left the room, the door was shut all the way. S2 stated that the door should be shut to secure the Water Treatment Room. During an interview at 11:50 AM, S1 stated that the doors should not have been “left like that” and should not be unsecured. Review of facility policy titled, “Central Water Treatment Systems Minimum Components and Requirement,” revised April 2025, stated, “... GENERAL REQUIREMENTS: ... 3. New and renovated water purification and distribution systems should be located in a secure area that is readily accessible to those teammates authorized by the dialysis facility. Access to the water treatment system should be restricted to those personnel/trained teammates responsible for monitoring, maintenance of the system or for ordering or obtaining supplies. ...””
