What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 335 words
“POC-ACHIEVE ADEQUATE CLEARANCE CFR(s): 494.90(a)(1) Achieve and sustain the prescribed dose of dialysis to meet a hemodialysis Kt/V of at least 1.2 and a peritoneal dialysis weekly Kt/V of at least 1.7 or meet an alternative equivalent professionally-accepted clinical practice standard for adequacy of dialysis. This Standard is not met as evidenced by: 17026 Based on record review and staff interview, the facility staff failed to follow the physician order for the prescribed dialysis treatment time or document the reason for stopping dialysis treatment early for three patients (Patient # 1, # 2, and # 3) out of three clinical records reviewed. This had the potential to effect all patients on dialysis and cause an increase in complications. The facility census was 92. Findings included: 1. Review of the treatment flow sheet dated 4/25/24 for Patient # 1 showed the physician directed staff to provide three hours and 30 minutes of dialysis treatment for the patient. Further review of the treatment flow sheet showed staff stopped the dialysis treatment after three hours and two minutes and did not document the reason for stopping dialysis treatment early. 2. Review of the treatment flow sheet dated 4/26/24 for Patient # 2 showed the physician directed staff to provide four hours of dialysis treatment for the patient. Further review of the treatment flow sheet showed staff stopped the dialysis treatment after three hours and 42 minutes and did not document the reason for stopping dialysis treatment early. 3. Review of the treatment flow sheet dated 4/27/24 for Patient # 3 showed the physician directed staff to provide four hours of dialysis treatment for the patient.
