What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“IC-SANITARY ENVIRONMENT CFR(s): 494.30 The dialysis facility must provide and monitor a sanitary environment to minimize the transmission of infectious agents within and between the unit and any adjacent hospital or other public areas. This Standard is not met as evidenced by: 17026 Based on observations and staff interview, the facility staff failed to maintain two water drains in the water room, failed to ensure the area under an exit door was sealed, and failed to ensure the laboratory area refrigerator remained clean. This had the potential to effect all patients on dialysis and cause an increase in complications. The facility census was 86. Findings included: 1. Observation on 8/11/25 at 11:15 AM of the water room showed an approximately one foot by one foot water drain near the water softener tanks that had rusted areas around the inside of the floor drain. 2. Observation on 8/11/25 at 11:17 AM of the water room showed an approximately one foot by one foot water drain in the back corner of the room that had black and yellow areas around the floor drain. 3. Observation on 8/11/25 at 11:19 AM of the storage room adjacent to the water room showed the exit door to the outside had an approximately one-fourth inch gap at the bottom of the door. 4. Observation on 8/11/25 at 11:22 AM of the laboratory area showed the refrigerator with laboratory supplies had black particles on the bottom shelf of the refrigerator. 5.
