What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Appropriateness Of Dialysis Rx
The inspectors’ words, 263 words
“PA-APPROPRIATENESS OF DIALYSIS RX CFR(s): 494.80(a)(2) The patient’s comprehensive assessment must include, but is not limited to, the following: (2) Evaluation of the appropriateness of the dialysis prescription, This Standard is not met as evidenced by: 17026 Based on record review and staff interview, the facility staff failed to follow the physician order for heparin (medication used to prevent blood clots) for three (Patient # 1, # 2, and # 3) patients out of three clinical records reviewed. This had the potential to effect all patients on dialysis and cause an increase in bleeding or blood clotting problems. The facility census was 79. Findings included: 1. Review of the treatment flow sheets dated 10/4/25 and 10/7/25 for Patient # 1 showed the physician directed staff to administer 2000 units of heparin midway through the dialysis treatment, but staff did not administer the 2000 units of heparin or document the reason. 2. Review of the treatment flow sheets dated 10/3/25 and 10/6/25 for Patient # 2 showed the physician directed staff to administer 6000 units of heparin midway through the dialysis treatment, but staff did not administer the 6000 units of heparin or document the reason. 3. Review of the treatment flow sheet dated 10/2/25 for Patient # 3 showed the physician directed staff to administer 6000 units of heparin midway through the dialysis treatment, but staff administered 7000 units of heparin. 4. During an interview on 10/21/25 at 11:35 AM, the clinic manager said staff should follow the physician order for heparin administration or document the reason for not following the physician order.”
