What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Environment: Secure And Restricted
The inspectors’ words, 364 words
“ENVIRONMENT-SECURE & RESTRICTED CFR(s): 494.40(a) 8 Environment: secure & restricted The water purification and storage system should be located in a secure area that is readily accessible to authorized users. The location should be chosen with a view to minimizing the length and complexity of the distribution system. Access to the purification system should be restricted to those individuals responsible for monitoring and maintenance of the system. This Standard is not met as evidenced by: 39083 Based on observation, interview, and record review, the facility failed to secure the water room, resulting in the potential for unauthorized access to the water room, resulting in the potential for harm to all patients in the facility. Findings include: On 4/23/24 at 11:20 AM, the janitorial supply closet and central storage area were observed to be located in the same enclosure of the water treatment room. At this time, Biomed “N” was queried on the vendor access to the water treatment area and stated that the contracted cleaning company has access to the janitorial closet, which in turn gives the cleaning company direct access to the water treatment area. During an interview on 4/23/24 at 11:38 AM, Area Technical Operations Manager (ATOM) “O” stated that there used to be a cage door to the treatment area, but has since been removed after renovations to the water treatment area. During an interview on 4/23/24 at 12:25 PM, Biomed “N” confirmed that the contracted cleaning company has access to the water treatment room while no other staff are in the facility. During an interview on 4/23/24 at 1:15 PM, Clinical Manager “A” stated that delivery vendors, biowaste collectors, and the janitorial service all have access to the water treatment room. According to the facility’s “Janitorial Contractor Authorization Form,” signed 11/9/2018, it notes, “Contractor Personnel are hereby authorized to be in treatment and storage areas that house medical equipment and medical systems (example: multipurpose storage rooms that also houses water purification and concentrate mixing systems) with the following exceptions: - Contractor Personnel will not access nor be responsible for cleaning secured rooms that are dedicated for water purification equipment and/or concentrate mixing and delivery equipment as designated by the facility...””
