What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Appropriateness Of Dialysis Rx

    The inspectors’ words, 391 words

“PA-APPROPRIATENESS OF DIALYSIS RX CFR(s): 494.80(a)(2) The patient’s comprehensive assessment must include, but is not limited to, the following: (2) Evaluation of the appropriateness of the dialysis prescription, This Standard is not met as evidenced by: 47759 Based on record review, the facility’s governing body failed to ensure that the dialysis prescriptions were the same as what the patients received. This failure placed the affected patients at potential risk of harm and denied patients their rights to a minimum standard of medical care. Findings included: According to the facility’s policy titled “Patient Assessment and Monitoring” (Reference Number 45284) page 6 stated, “check prescribed blood flow is being achieved or reason is documented in the medical record if unable to meet prescribed blood flow.” Page 4 stated, “Obtain blood pressure and pulse rate every 30 minutes or more as needed but not to exceed 45 minutes or per state regulations...Document machine parameters and safety checks every 30 or more often as needed but not to exceed 45 minutes or per state regulations.” On 09/05/2025, at 12:00 p.m. during record review of the selected patients’ treatment flowsheets, the surveyor observed that the prescribed treatment was not followed in 6 out of 15 treatment flowsheets audited. The following are examples of the deviations identified: 1. On treatment date 08/28/2025, Patient A had a prescribed blood flow rate (BFR) of 450 milliliter (mL)/minute(min). During record review of Patient A’s treatment flow sheet, the BFR was set at 400 mL/min from 08:20 a.m. -09:33 a.m. with no supporting documentation found explaining why the BFR prescription was not followed. 2. On treatment date 08/28/2025, Patient A had a treatment time prescribed for 3 hours and 45 minutes. Review of Patient A’s treatment flowsheet revealed documentation that indicated the patient dialyzed for only 3 hours. No supporting documentation was found explaining why the prescription was not followed. 3. On treatment date 08/30/2025, Patient B had a prescribed BFR of 450mL/min. This order was not met for the entire treatment. In addition, from 09:32 a.m., no blood pressure check or safety check was conducted until 10:36 a.m. On 09/05/2025 at 01:00 p.m., interviews conducted with the Clinical Manager stated that Staff members were instructed to follow the provider’s prescribed orders and if the orders cannot be achieved, then the Nurse must be notified and the reason must be documented.”
