What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Pe - Equipment Maintenance - Manufacturer’S Dfu

    The inspectors’ words, 345 words

“PE-EQUIPMENT MAINTENANCE-MANUFACTURER’S DFU CFR(s): 494.60(b) The dialysis facility must implement and maintain a program to ensure that all equipment (including emergency equipment, dialysis machines and equipment, and the water treatment system) are maintained and operated in accordance with the manufacturer’s recommendations. This Standard is not met as evidenced by: 30271 Based on observation, interview, and facility policy review, the facility failed to maintain facility furniture, counters, and dialysis machines in serviceable condition which had the potential to negatively impact the safety and wellbeing of the 27 patients who received in-center dialysis treatments at the facility. The findings include: Review of the facility policy titled, “Fresenius 2008 Series Diasafe Plus Filter Replacement,” revised 04/2025, revealed to remove “the clear protective cover from the Diafix Lock System on the back of the machine.” Further review of the policy revealed to reinstall “the clear protective cover.” During observation on 09/08/2025 at 9:30 AM, with the Facility Administrator (FA), revealed five out of the six chairs in the waiting room had worn areas with exposed fabric. During observation of the treatment floor on 09/08/2025 at 4:00 PM, revealed the surface of the counter on the water box area behind Stations 1, 2, 4, and 5 was broken at one corner. Per continued observation, two of the machines had the Diasafe Plus filter covers off which had been placed on the back shelf. Further observation revealed two additional machines were missing the Diasafe Plus filter covers. During observation of the treatment floor on 09/10/2025 at 10:50 AM, and interview with the FA, revealed the FA confirmed the Diasafe covers were either off or missing on the backs of four dialysis machines, Station 1, 2, 4, and 5. The FA confirmed an area on the countertop that was broken, and the boards were pushed down on one side of the countertop. In addition, the FA also confirmed five of the chairs in the lobby area were in disrepair. During interview on 09/11/2025 at 10:55 AM, the FA stated the facility’s policy had not been followed for the Diasafe covers being replaced.”
