What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Ic - Wear Gloves/Hand Hygiene
The inspectors’ words, 274 words
“IC-WEAR GLOVES/HAND HYGIENE CFR(s): 494.30(a)(1) Wear disposable gloves when caring for the patient or touching the patient’s equipment at the dialysis station. Staff must remove gloves and wash hands between each patient or station. This Standard is not met as evidenced by: 49050 Based on observation, interview, and review of the facility’s policy, the facility failed to ensure staff wore disposable gloves when caring for the patient or touching the patient’s equipment at the dialysis station for one observed patient. The findings include: Review of the facility’s policy titled, “[Facility] Infection Control Overview,” Version 4, dated 05/02/2022, indicated infection control policies included, but were not limited to: hand hygiene; dialysis unit precautions (including the use of personal protective equipment); and work practice controls: controls that reduced the likelihood of exposure by altering the manner in which a task was performed. Observation on 12/10/2024 at 10:56 AM revealed Registered Nurse (RN) 1 walked to Chair 12 at the dialysis station and grabbed the intravenous (IV) tubing connected to the IV bag with an ungloved right hand and pulled on the tubing. In an interview on 12/10/2024 at 11:09 AM with RN1, she stated she was not aware of grabbing the IV tubing with her bare hand. She stated, “We should have gloves on when touching the equipment at the dialysis station.” In an interview on 12/10/2024 at 3:19 PM with the Facility Manager, she stated, “RN1 came up to me and told me what she had done. RN1 stated she did not remember grabbing the tubing with her ungloved hand.” The Facility Manager stated, “RN1 knows they are to where gloves at the dialysis stations.””

Mixing Systems - Cleaning
The inspectors’ words, 265 words
“MIXING SYSTEMS-CLEANING CFR(s): 494.40(a) 6.4.1 Mixing systems: cleaning Concentrate mixing equipment should be either: (1) completely emptied, cleaned, and disinfected according to the manufacturer’s instructions; or (2) cleaned and disinfected using a procedure demonstrated by the facility to be effective in routinely producing concentrate meeting [these regulations related to allowable bacterial and endotoxin levels]. The disinfection data should be recorded for each ...disinfection cycle using a dedicated log. This Standard is not met as evidenced by: 16113 Based on observation and interview the facility failed to ensure the acid concentrate mixer was cleaned externally to prevent contamination of the acid concentrate. The findings include: Observation on 12/10/2024 at 10:55 AM of the acid concentrate mixer contained in the water room revealed the top of the acid concentrate mixer had large deposits of a white crystalline substance. The white crystalline substance also had brown areas. The white crystalline substance was surrounding the opening of the acid concentrate mixer posing a risk of contamination of the mixed acid concentrate. In an interview on 12/10/2024 at 11:57 AM with the Patient Care Technician (PCT) responsible for testing the water system that day, the PCT stated PCT staff rinsed the acid concentrate mixer weekly. However, the PCT stated the acid concentrate mixer appeared not to have been rinsed off for a while. In an interview on 12/10/2024 at 12:05 PM with the Area Technical Operations Manager (ATOM), the ATOM stated the technical staff members were responsible for cleaning the acid concentrate mixer on a monthly basis. The ATOM stated the acid concentrate mixer should have been cleaned more thoroughly.”
