What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Esrd Patient Orientation Training
The inspectors’ words, 285 words
“ESRD Patient Orientation Training CFR(s): The dialysis facility must provide appropriate orientation and training to patients, including the areas specified in paragraph (d)(1) of this section. This Standard is not met as evidenced by: 50529 Based on document review and interview, the facility failed to ensure that 2 of 5 sampled patients (Patient 1 and 3) were provided with appropriate orientation and training to ensure patient knowledge of emergency procedures for both in and out of the facility, upon admission, quarterly, annually, and with any changes and/or updates to the facility’s Emergency Preparedness Plan. The ESRD facility’s failure to provide patient education and training could inhibit safe evacuation during an emergency and has the potential to negatively impact the health and well-being of the 49 patients who received treatment at this facility. Findings Include: On 08/13/25 at 8:50 AM, chart review for Patients 1 and 3 failed to show documented evidence that patients were provided with information of the facility’s emergency preparedness policies and procedures that could be verified by patient signature of receipt and understanding and failed to provide patients with assigned evacuation locations in the event of an emergency. During an interview on 08/13/2025 at 12:45 PM, Staff A, Facility Administrator, confirmed that education on the emergency evacuation locations was not provided to Patients 1 and 3. Staff A stated that facility policy requires that all patients receive emergency preparedness training at admission, annually, and as needed; as well as emergency evacuation training reviewed every quarter. Emergency Preparedness training and forms should be completed to ensure patients have the most accurate information in an emergency. Failure to follow policy could potentially put patients at risk for injury or possible fatality in an emergency.”
