What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Patients Seen By Med Staff Member Monthly

    The inspectors’ words, 291 words

“POC-PTS SEEN BY MED STAFF 1X/MO CFR(s): 494.90(b)(4) The dialysis facility must ensure that all dialysis patients are seen by a physician, nurse practitioner, clinical nurse specialist or physician’s assistant providing ESRD care at least monthly, as evidenced by a monthly progress note placed in the medical record, and periodically while the hemodialysis patient is receiving in-facility dialysis. This Standard is not met as evidenced by: 27125 Based on document review and interview, it was determined that for 1 of 2 (Pt. #5) home peritoneal dialysis patients, the Facility failed to ensure each patient was seen and current status was addressed by the physician, at a minimum of monthly. Findings include: 1. The Facility’s policy titled, “Comprehensive Interdisciplinary Assessment and Plan of Care (7/3/2023)” was reviewed on 1/30/2024 and included, “At a minimum, monthly medical progress notes ... must be in every patient’s medical record that documents a physician ... has seen each patient.” 2. The clinical record of Pt. #5 was reviewed on 1/30/2024. Pt. #5 was admitted on 6/21/2023 for in center dialysis. Pt. #5 changed modalities on 9/18/2023 to peritoneal dialysis (PD)(same clinic). The monthly physician assessments, dated 11/20/2023 and December 12/9/2023 were identical in content and included that Pt. #5 was still on in center dialysis three times a week. The physician progress notes did not indicate the change to PD in the assessments. 3. During an interview on 1/30/2024 at approximately 10:30 AM, the Home Care Manager (E#2) stated that the physician has a pre filled document and the previous data is carried over. E#5 stated, “The physician is supposed to update the assessment each month with the current assessment of the patient. The assessments for [Pt. #5] were not changed or updated with current information.””

    Rule V0560. Standard-level.
