What these words mean2
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 171 words

“IC-HBV-VACCINATE PTS/STAFF CFR(s): 494.30(a)(1)(i) Hepatitis B Vaccination Vaccinate all susceptible patients and staff members against hepatitis B. This Standard is not met as evidenced by: 42316 Based on review of personnel files and staff interview, it was determined the facility failed to ensure hepatitis B antibody testing was completed for 1 of 2 Staff members (Staff A) whose personnel files were reviewed. Failure to test for hepatitis B antibodies created unknown hepatitis B susceptibility for the staff member. Findings include: Staff A’s personnel file was reviewed. Her most recent hepatitis B antibody testing was requested. The lab was drawn in May 2024 however, it was canceled, and Staff A’s hepatitis B susceptibility was unknown. It was unclear if Staff A was susceptible to hepatitis B. The FA was interviewed on 8/14/24 at 9:37 AM and Staff A’s personnel file was reviewed with him. He confirmed there was no current hepatitis B antibody test for Staff A on file. The facility failed to ensure Staff A was tested for hepatitis B antibodies.”
