What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Pe - Building - Constructed/Maintained To Ensure Safety
The inspectors’ words, 337 words
“PE-BUILDING-CONSTRUCT/MAINTAIN FOR SAFETY CFR(s): 494.60(a) The building in which dialysis services are furnished must be constructed and maintained to ensure the safety of the patients, the staff and the public. This Standard is not met as evidenced by: 32734 Based on observation and staff interviews, it was determined that the facility failed to ensure that the building in which dialysis services were furnished were maintained free from damage/defects and hazards to ensure safety and functionality as evidenced by damaged wall and baseboard in one of one Water Treatment Room and an obstructed exit door and damaged floor in one of one Supply Room. The failure to maintain the integrity of all surfaces, could hinder effective cleaning and disinfection of surfaces and could promote microbial growth. The obstructed exit door had the potential for all patients and staff to become trapped or significantly delayed during an emergency evacuation, that could lead to increased risk of injury due to fire, smoke inhalation, or other hazards, as a blocked exit pathway impedes the ability to quickly and safely leave a building. Findings include: During a tour of the facility, accompanied by the Clinical Manager (CM) on 10/10/24 between 10:30 a.m. and 12:30 p.m., the following was observed: In the Water Treatment Room: - The wall behind the acid holding tank, was bulging/ballooning outwards and crumbling that spanned over three feet wide by three feet high at just above the floor and the baseboard. - The baseboard was separated from the wall. In the Supply Room: - The exit door in the Supply Room was partially blocked by supplies in boxes on a wooden pallet, a cart that stored approximately 20 used 300 ml (milliliter) plastic bottles labeled “bleach” on the right side and two 100-gallon drums on the left of the door. - The floor had dark gray and brown erosion from paint finishes that were peeling off throughout the floor. During an interview on 10/10/24 at 12:35 p.m., the CM and Director of Operations both acknowledged the above findings.”
