What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

    The inspectors’ words, 347 words

“IC-DISINFECT SURFACES/EQUIP/WRITTEN PROTOCOL CFR(s): 494.30(a)(4)(ii) [The facility must demonstrate that it follows standard infection control precautions by implementing- (4) And maintaining procedures, in accordance with applicable State and local laws and accepted public health procedures, for the-] (ii) Cleaning and disinfection of contaminated surfaces, medical devices, and equipment. Patient #3 and Patient #7 Findings include: On 08/25/2025, at approximately 3:00 PM, Staff E/ CHT was observed cleaning Station #9 which had been previously used by Patient #3. The surveyor had previously noticed that some of the dialysis chairs had sides that appeared to open, though most did not. The surveyor asked Staff A /Assistant Administrator, “How many chairs do you have that open? Staff A /Assistant Administrator stated “I don’t think we have any that open”. Staff E/ CHT was observed opening the side wings of the dialysis chair, cleaned them with bleach and immediately closed the sides. The surveyor then asked Staff E/ CHT, “How long are you supposed to leave the sides open on the chair”? Staff E/ CHT replied “for 10 minutes”; he then stopped, turned around and opened the chair sides. On 08/27/2025, at approximately 10:00 AM, the surveyor had finished interviewing Patient #7, the surveyor noticed the chair had a locking mechanism on each side. Patient #7 left, Staff F/ CHT then went and cleaned the machine and the chair, but he did not open the sides. In the presence of Staff A/ Assistant Administrator and Staff G/ Director of Nursing, the surveyor waited for a few minutes and then washed his hands, put on gloves and unlatched the sides of the chair. Staff F/ CHT then went over and cleaned the insides of the chair. The surveyor asked Staff A/ Assistant Administrator and Staff G/ Director of Nursing for the facility’s policy and procedures regarding the cleaning of equipment several times, but it was never provided.”

    Rule V0122. Standard-level.

  • Implement Updates Of Poc - 15 Days P Pt Assessment

    The inspectors’ words, 234 words

“POC-IMPLEMENT UPDATE-15 DAYS P PT ASSESS CFR(s): 494.90(b)(2) Implementation of monthly or annual updates of the plan of care must be performed within 15 days of the completion of the additional patient assessments specified in §494.80(d). This Standard is not met as evidenced by: 47207 Based on medical record review and interviews, the facility failed to complete an unstable monthly Plan of Care (POC) report in a timely manner for 1 out of 10 sampled patients reviewed. Patient #1 Findings include: On 08/26/2025, a review of Patient #1’s medical record revealed that a POC was completed on 07/22/2025, however it had not been signed either electronically or in person by the patient and the Medical Director. At approximately 2:30 PM on 8/26/2025, Staff A /Assistant Administrator and Staff G /Director of Nursing were asked why Patient #1’s POC had not been completed. They stated that the physician had not e-signed his portion of the plan, if he had then they could have printed the form to have all staff physically sign the plan, and the Medical Director has been out of town but would be coming back on 8/30/2025. The surveyor then pointed out to Staff A /Assistant Administrator and Staff G /Director of Nursing that the physician had documented assessments on other patients in the facility after 07/22/2025, why did he not sign Patient #1’s POC then? They could not provide an answer.”
