What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Ic - Sanitary Environment

    The inspectors’ words, 376 words

“IC-SANITARY ENVIRONMENT CFR(s): 494.30 The dialysis facility must provide and monitor a sanitary environment to minimize the transmission of infectious agents within and between the unit and any adjacent hospital or other public areas. This Standard is not met as evidenced by: 13640 Based on observation and interview, the facility failed to ensure the provision of a sanitary environment beneath facility sinks to minimize the transmission of infectious agents. Findings: On 8/11/25 from 9:48 AM through 9:55 AM, a tour of the facility revealed discrepancies with the areas beneath three separate sinks on the treatment floor. One sink was positioned on the exterior of the isolation room and the other sink was positioned in the middle of three station islands in the middle of the treatment floor. The area beneath each sink was encased in a cabinet structure, each with two doors that had upper and lower hinges, left and right. The sink in the center island had a partition separating the sink drain pipe from the anterior portion of the cabinet structure, leaving a narrow ledge at the front of the cabinet. The area beneath the sink that was adjacent to the isolation room had soiled flooring. Upon the floor of the sink cabinet was a 3 inch by 3 inch mound of a white substance, just below a connector on the sink drain pipe. There was also an eight inch by four inch area of the floor which was covered by a black, powdery material. There was also a twelve inch by eight inch area of the floor covered by a brown stain. On 8/11/25 at 10:00 AM, the administrator confirmed the presence of the above-stated observations. The presence of such observed materials in the vicinity of the treatment floor may contribute to the grown of microorganisms that could have an adverse effect on patients. In addition, the storage of materials used in the overall patient care process in such areas would expose them to the risk of water seepage from the sink, which, in turn, could lead to the growth of microorganisms in the area. This risk, coupled with the presence of materials on the cabinet floor, as discussed above, would also pose an infection risk for patients in the treatment floor environment.”

    Rule V0111. Standard-level.
