What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Manage Volume Status
The inspectors’ words, 323 words
“POC-MANAGE VOLUME STATUS CFR(s): 494.90(a)(1) The plan of care must address, but not be limited to, the following: (1) Dose of dialysis. The interdisciplinary team must provide the necessary care and services to manage the patient’s volume status; This Standard is not met as evidenced by: 13640 Based on observation, interview, record review and a review of facility documentation, the facility failed to ensure that the necessary services in the form of a physician ordered blood flow rate were followed to maintain the patients volume status in one of eight sampled patients. (#8) Findings: During an observation of patient #8 on 1/12/26 at 10:57 AM, the patient’s blood flow rate, as establisherd on the hemodialysis machine, was at 400 ML (milliliters) / min (minute). A review of the physician orders at this time revealed that the ordered blood flow rate in effect for this date was at 450 ML/MIN. During an interview of the clinical manager at this time, she indicated that she could not find any documented reason in the medical record, up to that point, for the order to not have been followed. She stated that the order should have been followed. A review of the treatment sheet for patient #8 for 1/22/26 revealed that the hemodialysis treatment session began at 7:26 AM. A blood flow rate of 400 ML/MIN was documented at this time. It was maintained at this level through the terminationof treatment at 11:19 AM. There were no entries on the treatment sheet which gave an explanation for the orders for 450 ML/MIN not having been followed. A review of facility policy Initiation of Treatment Using an Arteriovenous Graft or Fistula revealed the following: “Start the blood pump and increase the blood flow slowly to 100-150 ML/MIN . . . If no problems are noted, turn up the blood flow rate prescribed by the physician to acheive optimal clearances.” The facility was not in compliance with this policy.”
