What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

  • Manage Volume Status

    The inspectors’ words, 358 words

“POC-MANAGE VOLUME STATUS CFR(s): 494.90(a)(1) The plan of care must address, but not be limited to, the following: (1) Dose of dialysis. The interdisciplinary team must provide the necessary care and services to manage the patient’s volume status; This Standard is not met as evidenced by: 45827 Based on treatment record reviews, policy review and interview, the facility failed to ensure abnormal blood pressures were reported to the licensed nurse for 2 of 9 reviewed dialysis treatments for Patient #4. The findings include: On 8/8/24 a review was conducted of 9 of Patient #4’s treatment records from 7/9/24 - 8/6/24. On 8/1/24, Patient #4 had 11 recorded intradialytic (occurring during hemodialysis) blood pressures with a systolic less than 90 millimeters of mercury (mm/Hg) and/or a diastolic less than 50mm/Hg. On 8/6/24, Patient #4 had 4 recorded blood pressures with a systolic less than 90mm/Hg and/or a diastolic less than 50mm/Hg. These blood pressure readings were recorded by Staff A/Patient Care Technician (PCT). No treatment record notes were found to indicate if the PCT notified the Registered Nurse (RN) of the abnormal blood pressures. On 8/9/24 a review of the facility’s Policy: 1-03-08, “Pre-Intra-Post Treatment Data Collection, Monitoring, and Nursing Assessment” was completed. On page 4, #11 indicates, “Abnormal findings or findings outside of any patient specific physician ordered parameters will be reported to the licensed nurse immediately (refer to “Abnormal Findings section in this policy). The licensed nurse will use his/her clinical judgement based on individual patient needs to determine if any clinical interventions are necessary. On page 6, under Blood Pressure-Intradialytic, it states, “Systolic less than 90mm/Hg, Diastolic less than 50mm/Hg.” On 8/8/24 at approximately 3:00 PM an interview was conducted with the Group Facility Administrator (GFA) who stated she was unable to locate any treatment notes to show Staff A/PCT notified a RN of Patient #4’s abnormal blood pressures. She stated the facility has strong PCTs that participate in rounds with the Medical Director. She further stated the facility policy is for the PCT to notify the RN of abnormal blood pressures. She confirmed there were no doctor ordered parameters for Patient #4’s low blood pressure.”

    Rule V0543. Standard-level.
