What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

The inspectors’ words, 277 words
“MR-COMPLETE, ACCURATE, ACCESSIBLE CFR(s): 494.170 The dialysis facility must maintain complete, accurate, and accessible records on all patients, including home patients who elect to receive dialysis supplies and equipment from a supplier that is not a provider of ESRD services and all other home dialysis patients whose care is under the supervision of the facility. This Standard is not met as evidenced by: 40861 Based on interview and record review, the facility failed to ensure the clinical record was accurate for one of two sampled patients (Patient 1) when the registered nurse’s Pretreatment Assessment was not documented in Patient 1’s treatment record. This failure had the potential to result in an inaccurate medical record and did not portray the actual care provided to Patient 1. Findings: During a review of the facility’s policy and procedure titled, Pre-Intra-Post Treatment Data Collection, Monitoring And Nursing Assessment, Revision Date October 2023, indicated in part... “2. The Nursing assessment will be performed and documented by a licensed nurse.” During a concurrent interview and record review on 2/27/24 at 12:48 p.m., with Facility Administrator (FA), Patient 1’s Treatment Details Report (TDR), dated 2/22/24 was reviewed. The TDR indicated the Treatment Nurse Assessment was incomplete for Patient 1 on 2/22/24. FA agreed that all fields of the assessment should have been completed by the nurse and were not. During a concurrent interview and record review on 2/27/24 at 13:10 p.m., with Licensed Nurse (LN1), Patient 1’s TDR, dated 2/22/24 was reviewed. The TDR indicated the Treatment Nurse Assessment was incomplete for Patient 1 on 2/22/24. LN1 agreed that all fields of the assessment should have been completed by the nurse and were not.”
