What these words mean2
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

“MR-COMPLETE RECORDS PROMPTLY CFR(s): 494.170(b)(1) (1) Current medical records and those of discharged patients must be completed promptly. This Standard is not met as evidenced by: 50568 Based on document review and staff interview, the Department determined that the facility failed to ensure that a documented discharge summary for patients was reviewed within 30 days of the patient’s discharge from the facility. The lack of timely medical record documentation poses the potential risk of lack of coordination and continuity of care for patients. Findings Include: Facility policy titled, “ Medical Record Quality Assurance” states that, “ ... A discharge note is to be entered into the medical record within 30 days of the patient’s discharge date ...” During the entrance conference on 07/08/2024, a request was made for patients who have been discharged from the facility within the previous 6 months. The following patients were selected from the documentation provided by the facility, with discharges occurring between the months of 01/2024 and 05/2024: Patient #5, Patient #6, Patient #7, and Patient #8. However, review of the documentation did not include a signed discharge summaries for each of these patients.
