The short answer

Record last released by CMS on Oct 28, 2026.

Each figure’s source sits beside it, in its section below.

What these words mean3
Star ratings
CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Findings include: Arizona Administrative Code requires: “...R9-10-1018. Dialysis Services ...E. An administrator of an outpatient treatment center that is authorized to provide dialysis services shall ensure that for a patient receiving dialysis services:...7. The patient:...e. Is monitored by a personnel member while receiving dialysis at least once every 30 minutes ...” The facility policy titled “Patient Assessment and Monitoring” requires “...during Treatment ...Obtain blood pressure and pulse rate every 30 minutes or more as needed but not to exceed 45 minutes or per state regulations. Document machine parameters and safety checks every 30 minutes or more often as needed but not to exceed 45 minutes or per state regulations ....” Patient #4 ’ s medical record review on 07/21/2025 revealed: Treatment sheet dated 07/17/2025 08:44 to 09:22 vital signs 38 minutes apart Treatment sheet dated 07/10/2025 09:00 to 09:42 vital signs 42 minutes apart Treatment sheet dated 07/05/2025 10:01 to 10:40 vital signs 38 minutes apart Employees #1, 2, and 5 confirmed on 07/21/2025 that Patient #4 did not have safety checks performed every 30 minutes according to facility policy.”
