What these words mean3
- Star ratings
- CMS rates some kinds of place from 1 to 5 stars, more stars meaning better on CMS’s own measures for that kind: for a nursing home, its health inspections, staffing and quality measures. A rating is CMS’s summary of those measures; it doesn’t promise good care. Source: CMS, Care Compare and the Five-Star Quality Rating System.
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.

Cartridge Filters - Config And Monitoring
The inspectors’ words, 411 words
“CARTRIDGE FILTERS-CONFIG & MONITORING CFR(s): 494.40(a) 5.2.3 Cartridge filters: config and monitoring The cartridge is contained within an opaque filter housing with seals to separate the feed and product water streams. When the maximum [pressure drop] ?P recommended by the filter manufacturer is reached, the cartridge should be replaced according to the manufacturer ’ s instructions. 6.2.3 Cartridge filters Cartridge filters should be monitored on a periodic .... basis for a [pressure drop] ?P across the filter [that] can be used to determine when the filter is retaining particulate matter to the point that the filter will no longer allow the required water flow without an excessive reduction in pressure at the outlet of the filter. A marked decrease in ?P without a corresponding decrease in flow rate may indicate a loss of filter integrity. Follow the manufacturer ’ s recommendations concerning when to replace cartridge filters. Replacement of the cartridge will usually be indicated by an increase in ?P to some specified value. A log sheet should be developed to record the pressure drop measurements. This Standard is not met as evidenced by: 45999 Based on a review of facility policy, daily water treatment logs, and staff interview, the Department determined that the facility failed to ensure staff notify the facility administrator (FA) and/or biomed technician (BMT) and appropriate action was taken when the prefilter pressure test result was not within the acceptable limits specified on the daily water treatment log for two (2) of three (3) months reviewed. Failure to maintain water logs per facility policy poses the potential risk of harm to patients and hospitalization if the water system were to malfunction. Findings include: Daily water treatment logs revealed noted issues with the pressure of the prefilter and documented staff notification to BioMed on the following dates: April 19, 2024 April 23, 2024 April 30, 2024 May 5, 2024 May 7, 2024 May 8, 2024 May 9, 2024 Observation of the water room on June 11, 2024 revealed multiple leaks coming from the booster pump, the RO system, and the booster pump to the loop for the treatment floor. A policy was requested, but none was provided. Employee #3 confirmed in an interview conducted on June 11, 2024, that appropriate action was not taken based on continued prefilter pressure test result outside of acceptable limits and multiple leaks in the water. Employee #3 further confirmed that not ensuring appropriate action was taken fails to align with facility policy.”

Pe - Safe, Functional, Comfortable Environment
The inspectors’ words, 249 words
“PE-SAFE/FUNCTIONAL/COMFORTABLE ENVIRONMENT CFR(s): 494.60 The dialysis facility must be designed, constructed, equipped, and maintained to provide dialysis patients, staff, and the public a safe, functional, and comfortable treatment environment. This Standard is not met as evidenced by: 51021 Based on review of facility policy, observation, and staff interview, the Department determined that the facility failed to ensure medications, supplies, and equipment have not expired. Failure to ensure that medication, supplies and equipment have not expired has the potential risk of using expired supplies and equipment for patients which could lower the efficacy of medication and/or could expose patients to supplies with decreased sterility; thus increasing their risk for infection. Furthermore, expired emergency supplies found inside of emergency carts could delay emergency treatment. On 06/10/2024, tour of the facility resulted with the following expired medical supplies: 1 (one) Vacuette NH Trace Elements Sodium Heparin test tube; expired 4/10/2024 2 (two) Serim Guardian Test Strips Bottles; expired 03/31/2024 2 (two) BD Precision Glide Needles 22g x1; expired 01/31/2023 181(one hundred and eighty one) BD Precision Needles 21g x1; expired 08/31/2023 5 (five) Cobas PCR Media uni Swab Packets; expired 04/30/2024 12 (twelve) Greiner Tubes For Solution Swab Sample Collection,Transportation & Storage. Saline 0.85% Solution 3.0ml; expired 05/2024 65 (sixty-five) BD 1ml Insulin Syringe U-100 Slip Tip With BD Precision Glide Needles; expired 04/30/2024 During an interview conducted on June 11, 2024, Employee #3 confirmed the expired supplies had not been discarded in accordance with manufacturer instructions, expiration dates or facility policy.”

Pe-Fire Safety-Life Safety Code
The inspectors’ words, 286 words
“PE-FIRE SAFETY-LIFE SAFETY CODE CFR(s): 494.60(e)(1) (1) Except as provided in paragraph (d)(2) of this section, dialysis facilities that do not provide one or more exits to the outside at grade level from the patient treatment area level must comply with provisions of the Life Safety Code (NFPA 101 and its Tentative Interim Amendments TIA 12-1, TIA 12-2, TIA 12-3, and TIA 12-4) applicable to Ambulatory Health Care Occupancies, regardless of the number of patients served. This Standard is not met as evidenced by: 50568 Based on observation and staff interview, the Department determined that the facility failed to ensure that a sprinkler system inspection had occurred in the past 12 months. The lack of annual inspection poses the potential risk that patients and staff would not be protected in the event of a fire if safety controls were to malfunction. Findings include: The facility policy titled “FACILITY EMERGENCY MANAGEMENT PLAN” requires: “ ...Fire Safety Preparedness 1. FA, or designee, is responsible to: a. Maintain a fire safety preparedness program to prevent or minimize the likelihood of a fire that may result in death, injury, or property damage in accordance with CMS and OSHA regulations and National Fire Protection Association (NFPA) and applicable local and state life safety code requirements ....” Observation during a tour on June 10, 2024, revealed the facility had a sprinkler system present. Documentation of a sprinkler system inspection conducted within the past 12 months was requested. No sprinkler system documentation for the past 12 months was provided. During an interview conducted on June 11, 2024, Employee #3 confirmed that a sprinkler system is present at the facility and no documentation for an inspection performed in the past 12 months was available.”

Assess B/P & Fluid Management Needs
The inspectors’ words, 1,952 words
“PA-ASSESS B/P, FLUID MANAGEMENT NEEDS CFR(s): 494.80(a)(2) The patient’s comprehensive assessment must include, but is not limited to, the following: Blood pressure, and fluid management needs. This Standard is not met as evidenced by: 45999 Based on a review of facility policy, medical records, and staff interviews, it was determined that the facility failed to require clinical staff to: 1. It was determined that the facility failed to require clinical staff to notify the nurse and the nurse provided a documented intervention for 4 out of 4 patients reviewed when the patient’s blood pressure (BP) and/or heart rate (HR) was out of range, per the facility policy, which poses the potential risk of cardiovascular compromise and hospitalization. 2. It was determined the facility failed to ensure that vital signs were monitored every 30 minutes while receiving dialysis by a personnel member for 6 (six) out of 8 (eight) patients reviewed, patients #1, # 4, #5, #6, #7 and #8. Failure to ensure patients are monitored by a personnel member every 30 minutes while on dialysis can result in a patient having an undetected adverse event. Findings include: 1. Facility policy titled “PRE-INTRA-POST TREATMENT DATA COLLECTION, MONITORING, AND NURSING ASSESSMENT” requires “...Vital signs and treatment monitoring ...the following are considered abnormal findings and should be reported to the licensed nurse and documented in the patient’s medical record...Intradialytic: Systolic greater than 180mm/Hg or less than 90mm/Hg ...Diastolic greater than or equal to 100mm/Hg or less than 50mm/Hg...Heart or Pulse Rate Pre/Intra/Post: Less than 60 beats per minute or greater than 100 beats per minute...”. Standing patient orders require provider notification with decrease of 20mm/Hg in Blood Pressure. A medical record review of 8 patient’s treatment sheets revealed all (7/8) patients had multiple occurrences, of blood pressure out of range without documentation of the licensed nurse being notified. From the facility
The public record above comes first. Anything the facility adds will appear here, below it, and say who wrote it.
Replies
None. Replies aren’t open yet. When they open, the facility will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.
Paid listing
None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the facility”, and paying will never change a finding, a rating or the order of results.
