What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

“The most recent inspection document were not posted.”

    Rule 22.1-289.011-F.

May 8, 2025: inspection1 deficiency
  • The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the center administers only those drugs that were dispensed from a pharmacy and maintained in the original, labeled container.

“Medication for Child #1 was not stored in the original, labeled container.”

    Rule 8VAC20-780-510-I.

May 1, 2024: inspection1 deficiency
  • Based on record review, the center failed to implement a monthly practice evacuation drill.

“Evidence: The center did not document a practice evacuation drill for December, 2023.”

    Rule 8VAC20-780-550-D.

Mar 7, 2024: complaint inspection2 deficiencies
  • Based on review of video the center failed to ensure that supervising staff of children shall always ensure their care, protection, and guidance as required.

    The inspectors’ words, 113 words

“Evidence: On 2/29/24 staff #1 was seen on video sitting in a chair in the area used by the children (gymnasium) while 4 children were fighting, 2 boys and 2 girls. The video showed that the staff member was positioned to see this fight and did not intervene but continued to stay seated. The fight started at approximately 4:51pm and continued until approximately 4:55pm; according to the time on the video. Staff #1 did not ensure the care, protection, and guidance of these children during this as she did not intervene as required. The fight was not addressed in any manner until staff #2 returned to the gym and intervened at approximately 4:55pm.”

    Rule 8VAC20-780-340-A. From a complaint.

  • Based on review of video the center failed to ensure that children under 10 years of age are always in sight and sound supervision as required.

    The inspectors’ words, 164 words

“Evidence: 1. On 2/29/24 staff #1 could not see children who had built a “fort” by using tumble mats that they positioned vertically in a tall standing circle. Staff #1 was seen on video sitting near the wall across the gym from this and could not see into this fort from her position. She did not get up and go over to the fort to check on the children; and did not provide sight supervision to these children. 2. A little while later, after the vertical “fort” was taken down by the children, another mat fort was built beside staff #1, who was still seated in the same place, which prevented her from seeing a small group of children that were to her right in the corner of the gym. Since she remained seated for the majority of the programs operation, she could not provide the required sight supervision of those children. Staff #2 was in and out of the gym during these times.”

    Rule 8VAC20-780-340-F. From a complaint.

Dec 13, 2023: inspection1 deficiency
  • Based on review of medication the center failed to ensure that medication had not expired as required.

“Evidence: The medication (Benadryl) for child A had expired as of 02/2023. This medication was in the active medication box to be administered in case of allergic reaction.”

    Rule 8VAC20-780-510-I.

Nov 29, 2022: inspection2 deficiencies
  • Based on observations and discussions with staff the center failed to ensure that children washed their hands before snack.

“Evidence: Children were observed to immediately enter the program, grab their snack bags and proceed to have snack without washing their hands as required. Children’s hands are required to be washed before and after snack.”

    Rule 8VAC20-780-500-A.

  • Based on review of documentation and discussions with staff the center failed to ensure that monthly emergency escape drills are conducted.

“Evidence: Discussions with staff and review of documentation showed that there was no September 2022 emergency escape drill conducted as required.”

    Rule 8VAC20-780-550-D.

    Rule 8VAC20-780-245-J-3.

  • 8VAC20-780-260-A

“Based on record review, the center did not have an up to date annual fire inspection. Evidence: 1. The last fire inspection was 10/29/2020.”

    Rule 8VAC20-780-260-A.

  • 8VAC20-780-340-I

    The inspectors’ words, 93 words

“Based on observation, each child was not greeted upon arrival at the center. Evidence: 1. The inspector observed that center staff had not arrived in the cafeteria until 3:40 p.m. Children enrolled in the YMCA after-school program are supposed to be dismissed to the cafeteria at 3:05 p.m. Because no YMCA staff was present, children were kept in their classrooms at first and then in the cafeteria and supervised by school staff until YMCA staff showed up. What the record doesn’t say

  • Virginia doesn’t grade how serious a violation is, so no severity is shown.
  • Virginia publishes a complaint’s allegations and findings only when its inspection found a violation and the records are linked.
  • Virginia doesn’t publish staffing levels or owners for individual providers.
  • A plan of correction sent late, or not at all, isn’t online: Virginia says to ask the inspector.
  • Virginia’s pages show about the last five years of inspections; for older ones, Virginia says to contact the licensing inspector.
  • Virginia says this information may not be accurate, up to date or complete, and shouldn’t be the only source in choosing care.

The public record above comes first. Anything the provider adds will appear here, below it, and say who wrote it.

Replies

None. Replies aren’t open yet. When they open, the provider will be able to answer any finding for free, and the reply will show beside that finding, unedited and dated.

Paid listing

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the provider”, and paying will never change a finding, a rating or the order of results.
