What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Quality rating
Many states rate child care in a quality rating system that goes past the licensing rules, such as Texas Rising Star or Kentucky All STARS. Taking part is often voluntary, so a program with no rating hasn’t been rated low. Source: Each state’s quality rating system.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.

  • Supervision

    The inspectors’ words, 127 words

“1713.A.&B.&C.: Based on observation at approximately 2:00 p.m., S2 failed to provide supervision to children at all times. The Specialist rang the doorbell of the center to gain entry. S2 came to the door, saw the Specialist through the window, and then left the door without opening it. Upon gaining entry to the center the Specialist identified that S2 was responsible for four children (C1, C2, C3, and C4) ages, three to eleven-months-old. S2 identified that no one was in her classroom when she came to the door. The center had adequate staff to provide the Specialist entry to the center and the S2 did not need to come to the door. S2 returned to the children correcting the supervision prior to the Specialist entering the center.”

    Rule 1713.A.&B.&C.

  • Infants - Car Seats

Specialist observed that C1, 3-months-old, was asleep in a baby swing and not in her assigned crib. There was no written authorization for sleeping for C1. S2 removed C1 from the swing and placed her in her crib prior the Specialist leaving the room.”

What the record doesn’t say

  • Each deficiency here is from the file of deficiencies the Department of Education sent in answer to a public records request, covering Jan 1, 2023 to Oct 19, 2026, as it wrote them. The file lists only deficiencies, so a visit that found none isn’t in it; the School Finder’s visits are listed below.
  • The Department’s file doesn’t grade deficiencies by how serious they are. Its statements of deficiencies, the reports themselves, open from Louisiana’s own page for this provider.
  • Louisiana lists only recent visits. In its words: “Inspection displayed on this page are the last 15 visits or last visits done within 3 years period of time.”
  • Louisiana also says: “Inspections conducted before July 2024 that do not have deficiencies may not have an available Inspection Report.”
  • Louisiana’s School Finder doesn’t show owners, staffing levels or enforcement actions for individual providers.
