The short answer
Record last released by Louisiana Department of Education on Oct 27, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Quality rating
- Many states rate child care in a quality rating system that goes past the licensing rules, such as Texas Rising Star or Kentucky All STARS. Taking part is often voluntary, so a program with no rating hasn’t been rated low. Source: Each state’s quality rating system.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.

Supervision
The inspectors’ words, 129 words
“1713.A.B.: Based on interviews at 12:27 p.m., S7 failed to ensure children were supervised at all times. On 07/13/2026, C1,1-year-old, C2, 1-year-old, and C4, 1-year-old, were left unsupervised in the classroom for approximately 1-2 minutes sometime between 10:45 a.m. - 11:00 a.m. S7 exited the classroom with C3, 1-year-old, to change his diaper at the changing table down the hallway, then returned with C3 once his diaper was changed. S7 asked S10 to supervise the classroom during the diaper change; however, S10 supervised from the hallway instead of in the classroom. Although the center does have half walls, S10 needed to be in the classroom to appropriately supervise the children. C1, C2 and C4 were not harmed or injured while unsupervised. This could not be corrected during the inspection.”

What the record doesn’t say
- Each deficiency here is from the file of deficiencies the Department of Education sent in answer to a public records request, covering Jan 1, 2023 to Oct 19, 2026, as it wrote them. The file lists only deficiencies, so a visit that found none isn’t in it; the School Finder’s visits are listed below.
- The Department’s file doesn’t grade deficiencies by how serious they are. Its statements of deficiencies, the reports themselves, open from Louisiana’s own page for this provider.
- Louisiana lists only recent visits. In its words: “Inspection displayed on this page are the last 15 visits or last visits done within 3 years period of time.”
- Louisiana also says: “Inspections conducted before July 2024 that do not have deficiencies may not have an available Inspection Report.”
- Louisiana’s School Finder doesn’t show owners, staffing levels or enforcement actions for individual providers.
