The short answer
Record last released by Louisiana Department of Education on Oct 27, 2026.

What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Quality rating
- Many states rate child care in a quality rating system that goes past the licensing rules, such as Texas Rising Star or Kentucky All STARS. Taking part is often voluntary, so a program with no rating hasn’t been rated low. Source: Each state’s quality rating system.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.

“1507.B.: Based on record review and interview at 1:00 p.m. S1 failed to maintain documentation of a daily attendance record for all Staff and Owners, to include their time of arrival and departure. The Specialist requested to review attendance documentation for S1, S2, and S6 for 12/02/2025, and no documentation was available. S1, S2 and S6’s attendance logs could not be retrieved due to the attendance records being kept at the Head Start main office.”

Supervision
The inspectors’ words, 130 words
“1713.A.B.C.: Based on record review and interviews on 01/06/2026 at 1:00 p.m., S1 failed to ensure that children were supervised at all times. On 12/02/2025, at around 8:06 a.m., C1, 3-years-old, was dropped off in the carpool line and walked unsupervised through the school to his classroom. When C1 reached his class, no Head Start staff was present in the classroom to receive him. At 8:12 a.m., S6 walked into the 3 to 4-year old classroom and discovered C1 alone in the classroom, and remained in the classroom with him until S5 arrived. Review of camera footage found that C1 was left unsupervised for approximately 6 minutes. S1 stated there was a change to the morning arrival procedure due to O2 being absent which led to a gap in supervision.”

What the record doesn’t say
- Each deficiency here is from the file of deficiencies the Department of Education sent in answer to a public records request, covering Jan 1, 2023 to Oct 19, 2026, as it wrote them. The file lists only deficiencies, so a visit that found none isn’t in it; the School Finder’s visits are listed below.
- The Department’s file doesn’t grade deficiencies by how serious they are. Its statements of deficiencies, the reports themselves, open from Louisiana’s own page for this provider.
- Louisiana lists only recent visits. In its words: “Inspection displayed on this page are the last 15 visits or last visits done within 3 years period of time.”
- Louisiana also says: “Inspections conducted before July 2024 that do not have deficiencies may not have an available Inspection Report.”
- Louisiana’s School Finder doesn’t show owners, staffing levels or enforcement actions for individual providers.
