The short answer
Record last released by Louisiana Department of Education on Oct 27, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Quality rating
- Many states rate child care in a quality rating system that goes past the licensing rules, such as Texas Rising Star or Kentucky All STARS. Taking part is often voluntary, so a program with no rating hasn’t been rated low. Source: Each state’s quality rating system.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.

The inspectors’ words, 159 words
“1103.A.5.C.1.2.D.: Based on interviews at 11:52 a.m., S1 failed to notify Licensing and Child Welfare within 24 hours of being made aware of the following critical incident on 5/21/2026, at 4:06 p.m.: On 5/21/2026, C1, 7-months-old, had a well checkup with her pediatrician. As O1, parent, was getting her undressed, C1 was observed to have spots on her belly that looked like bruises. She had close to 10 small red and purple spots on her belly that looked like fingertip bruises. O1 said the marks were not on the child this morning when she got the child ready for daycare. The daycare did not report anything happening to C1 either. There are concerns that something happened to C1 while she was at daycare. O1 has been in contact with the daycare’s owner about the marks. C1 was sent to Northern LA Medical Center in Ruston for labs and X-rays to assess her and to be on the safe side.”

What the record doesn’t say
- Each deficiency here is from the file of deficiencies the Department of Education sent in answer to a public records request, covering Jan 1, 2023 to Oct 19, 2026, as it wrote them. The file lists only deficiencies, so a visit that found none isn’t in it; the School Finder’s visits are listed below.
- The Department’s file doesn’t grade deficiencies by how serious they are. Its statements of deficiencies, the reports themselves, open from Louisiana’s own page for this provider.
- Louisiana lists only recent visits. In its words: “Inspection displayed on this page are the last 15 visits or last visits done within 3 years period of time.”
- Louisiana also says: “Inspections conducted before July 2024 that do not have deficiencies may not have an available Inspection Report.”
- Louisiana’s School Finder doesn’t show owners, staffing levels or enforcement actions for individual providers.
