What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Quality rating
Many states rate child care in a quality rating system that goes past the licensing rules, such as Texas Rising Star or Kentucky All STARS. Taking part is often voluntary, so a program with no rating hasn’t been rated low. Source: Each state’s quality rating system.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.

  • Critical Incidents and Required Notifications

    The inspectors’ words, 140 words

“1103.A.4.5.B.3.: Based on record review and interview at 1:00 p.m., S1 failed to immediately notify the Parent of the following critical incident: On 6/16/2025, at 8:00 a.m., S35 informed S36 that she heard a slap noise and C1 (three-years-old) began crying in her classroom as S28 watched over her classroom when she stepped outside for a few minutes. S35 thought S28 slapped C1, but she did not see anything. S36 stated C1 informed her that S28 hit him, but S28 stated she did not. S28 stated C1 hit the floor making a slapping sound. O1, C1’s father, was called at 11:54 a.m. and 1:22 p.m. O2, C1’s mother, was called at 1:22 p.m., and a voicemail was left. O1 returned the call at 1:30 p.m., and was provided details of the incident. The Specialist was unable to validate the allegation.”

What the record doesn’t say

  • Each deficiency here is from the file of deficiencies the Department of Education sent in answer to a public records request, covering Jan 1, 2023 to Oct 19, 2026, as it wrote them. The file lists only deficiencies, so a visit that found none isn’t in it; the School Finder’s visits are listed below.
  • The Department’s file doesn’t grade deficiencies by how serious they are. Its statements of deficiencies, the reports themselves, open from Louisiana’s own page for this provider.
  • Louisiana lists only recent visits. In its words: “Inspection displayed on this page are the last 15 visits or last visits done within 3 years period of time.”
  • Louisiana also says: “Inspections conducted before July 2024 that do not have deficiencies may not have an available Inspection Report.”
  • Louisiana’s School Finder doesn’t show owners, staffing levels or enforcement actions for individual providers.

None. Paid listings aren’t open yet. When they open, anything paid for will say “Paid by the provider”, and paying will never change a finding, a rating or the order of results.
