The short answer
Record last released by Delaware Office of Child Care Licensing on Nov 4, 2026.

What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Oct 6, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On September 17, 2025, the Office of Child Care Licensing received a complaint regarding a lack of positive behavior supports, a lack of supervision, and a lack of respect. An investigation was conducted on September 17, 2025. There is no evidence to substantiate the allegations of a lack of positive behavior supports and not communicating respectfully because all staff interviewed denied this. There is no evidence to substantiate the allegation of a lack of supervision because all staff and children interviewed stated that no child was ever left alone. |
| Sep 5, 2025 | OCCL Standards Complaint: Substantiated | On August 13, 2025, the Office of Child Care Licensing received a complaint regarding screen time, not following the schedule, lack of positive behavior supports, smoking, and lack of supervision or staff-to-child ratio when the staff leaves the classroom. An investigation was conducted on August 25, 2025. The complaint is substantiated for staff-to-child ratio because Staff #2 and Staff #3 left Staff #4, an aide, alone in the gym with up to 38 school-age children. Staff #3 also left the gym to retrieve two children from the lobby, leaving 38 children in the gym with one qualified staff member and one aide. Staff #5 stated she had left the classroom, and it was out of ratio when she left. The complaint is substantiated for not following the schedule because all morning the children were in the gym and the only classroom that was supposed to be in the gym was the Scott classroom. There is no evidence to substantiate the allegation of not implementing positive behavior supports because Staff #5 stated what procedures are used when undesired behavior is exhibited and all the interviewed children stated they have not and have not seen their friends yelled at or yelling at the face. There is no evidence to substantiate the allegation of staff providing vapes to children or smoking on the premises during hours of operation because all staff and children interviewed denied seeing smoking or vapes being provided to children. There is no evidence to substantiate regarding screen time requirements because although the television was on during the visit, staff denied the allegation of watching television all day. LS #1 explained the screen time regulation, and when LS #1 returned to the gym, the television was turned off. Additional concerns were observed, including the maximum group size for school-age children being exceeded, with up to 45 children in the gym and unqualified staff alone with school-age children in the lobby and the gym. In addition, Staff #6 di d not have a complete file on-site. |
| Jul 7, 2025 | OCCL Standards Complaint: Substantiated | On June 11, 2025, the Office of Child Care Licensing received a complaint regarding a lack of supervision, no permission for children to walk from the bus stop to the center alone, and a lack of positive behavior support. An investigation began on June 13, 2025, and concluded on June 16, 2025. The complaint is substantiated for a lack of supervision because Person #1 and Staff #4 stated there were no staff members present at the bus stop to meet the children. The complaint is substantiated for a lack of positive behavior support because staff stated Staff #4 told a child they would shove a pool stick up his a**. There is no evidence to substantiate the complaint for not having permission for children to walk from the bus stop to the center alone because although the children walked alone, it was a supervision issue and the facility s policy is the children are not allowed to walk from the bus stop without an adult. |
| Feb 1, 2024 | OCCL Standards Complaint: Substantiated | The Office of Child Care Licensing received a complaint on December 8, 2023, regarding positive behavior supports, staff communication, accident injury procedures, and staff participating in other activities when caring for children. An investigation was conducted on December 27, 2023. This complaint is substantiated for staff communication because staff admitted to arguing in front of children. There is no evidence to support the allegations of lack of positive behavior supports, accident injury procedures, and staff participating in other activities because interviewed staff denied these allegations. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
