The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Oct 20, 2026 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | The Office of Child Care Licensing received a complaint on October 2, 2026, regarding accident/injury procedures. An investigation was conducted on October 13, 2026. There is no evidence to substantiate this complaint for improper notification procedures for a head injury because Administrator #1 denied any child received a head or face injury in September and no families made them aware of any such injuries. A concern noted was a lack of supervision because upon the specialists arrival to the facility, Staff #1 was standing in the great room while children were in care in Classroom 8, leaving them alone. |
| Sep 17, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing (OCCL) received an accident report from the facility for Child #1 on August 18, 2026, and a complaint on August 19, 2026, regarding accident/injury procedures, and lack of supervision. A review of paperwork began on August 18, 2026, and the complaint investigation began on August 19, 2026, and concluded on August 27, 2026. There is no evidence to substantiate this complaint for lack of supervision because according to video footage, the child was supervised. There is no evidence to substantiate this complaint regarding accident/injury procedures because Administrator #1 contacted Parent #1 about the incident by telephone within 15 minutes of the incident, after administering care for a scrape, and the required documentation was provided. Administrator #1 also contacted LS #1 after hours on August 18, 2026, by e-mail, providing the accident injury report and various updates the following day. |
| Apr 12, 2024 | OCCL Standards Complaint: No Evidence to Substantiate with Concern | On March 18, 2024, a complaint was called into the Child Abuse and Neglect Report line and not accepted for investigation by the Institutional Abuse Unit (IAU). The Office of Child Care Licensing (OCCL) received the complaint on March 18, 2024, and accepted it for investigation regarding improper discipline and notification to OCCL. An investigation was conducted on March 22, 2024. There is no evidence to substantiate this complaint for failure to provide notification to OCCL because OCCL was notified of the injury as required. There is no evidence to substantiate the allegation of improper discipline because according to video footage, the administrator was observed interacting with the children and playfully tapping and pushing them with a book. The children were smiling during the interactions. Concerns noted were the incident report provided to the parents and OCCL did not contain accurate information on how the injury occurred. The incident report stated Child #1 lost his balance on the chair, but the video footage shows he was knocked over out of the chair as a result of the administrator s actions. Non-Compliance Cited: Reg #26: Ensure staff provide truthful information to parents and OCCL. The incident report stated Child #1 lost his balance on the chair, but the video footage shows he was knocked over out of the chair as a result of the administrator s actions. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
