The short answer
Record last released by Delaware Office of Child Care Licensing on Oct 23, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Mar 3, 2026 | OCCL Standards Complaint: Substantiated | On February 11, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding health exclusions, not providing truthful information, and caring for children in unapproved space. An investigation was conducted on February 18, 2026. This complaint is substantiated for caring for children in an unapproved space, because according to Administrator #1, usually two children are brought up to the front of the center near the offices for the last 10 minutes of every day to await parent pickup. There is no evidence to substantiate the allegation the center did not provide truthful information on the daily logs. According to Staff #1, who was the staff person in the infant room on February 9, 2026, Child #1 arrived with a stuffy nose, was coughing and sneezing, but Parent #1 did not write this on the form at drop off, therefore, Staff #1 wrote it. LS #1 also confirmed it was written on the attendance record under the health check section. There is no evidence to substantiate the allegation the center did not follow their health exclusion policies, because according to Staff #1, Child #1 s vomit on February 9, 2026, was not related to illness but rather their diagnosed reflux. Staff #1 stated Child #1 frequently spits up large quantities of their bottles. According to messages reviewed between Parent #1 and Staff #1, Parent #1 did not administer the proper dosage of medication to Child #1 the weekend before the incident, causing increased spit up. |
| Feb 19, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On January 31, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding accident/injury procedures, expulsion policy, positive behavior management, improper staff-to-child ratio, and not using respectful communication. An investigation was conducted on February 6, 2025. There is no evidence to substantiate this complaint because according to an accident/injury report reviewed, Child #1 s parent/guardian was made aware of the injury resulting in scratches on his/her face. Additionally, Staff #1 stated Child #1 s lips were very chapped and would crack and bleed, causing the bloody lip which the Reporter alleged to be an injury. There is no evidence to substantiate the allegation Child #1 was terminated without proper notice, because according to Administrator #1, she met with the parent/guardian twice, explaining after three more incidents of Child #1 hurting other children, his/her enrollment would be terminated. Additionally, according to the center s expulsion policy in the parent handbook, they can terminate a child s enrollment at any time, for any reason. There is no evidence to substantiate the allegation the center places children in excessive time-outs because staff denied this allegation. There is no evidence to substantiate the allegation the center not following proper staff-to-child ratio because according to the attendance records reviewed, the center maintained staff-to-child ratio. There is no evidence to substantiate the complaint the administrator was disrespectful to a parent/guardian because Administrator #1 denied this and according to Staff #2, whose classroom is closest to the front office, she has never heard Administrator #1 being disrespectful to any parents or guardians in the building. |
| Nov 27, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On November 6, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding water being unavailable to children. An investigation was conducted on November 20, 2024. There is no evidence to substantiate this complaint because all staff interviewed stated the children bring their own water bottles to the center and they are accessible to children in the classroom and refilled as needed. Additionally, Staff #2 stated the classroom staff carry each child s water bottle in a large bucket outside for outdoor play. While at the center, Staff #1 and Staff #2 confirmed Child #1 had a contagious skin infection called molluscum. When asked how the center handled this, all staff interviewed stated they kept the rash covered with bandages or clothing, in accordance with the doctor s note in Child #1 s file. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
