What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

DateWhatDetail
Aug 5, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernOn June 25, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding the suspension/expulsion policy. An investigation was conducted on July 15, 2026. There is no evidence to substantiate this complaint because the facility documented the incident through an Accident/Injury Report and a Behavior Incident Report, provided first aid, contacted OCCL to notify them of the injury, and imposed a documented five-day suspension on Child #2 following the center s suspension policy. A concern noted was according to staff interviews and the Accident/Injury Report, Child #1 received a head injury at 3:30 pm and Child #1 s parents were not notified until 4:45 pm at pick-up.
Jul 28, 2026OCCL Standards Complaint: SubstantiatedOn July 13, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of positive behavior supports, improper staff-to-child ratio, lack of supervision, improper sanitation, accident/injury procedures, and not using age-appropriate music. An investigation was conducted on July 15, 2026. The complaint is substantiated for improper staff-to-child ratio and improper sanitation. According to the attendance record for July 8, 2026, the preschool classroom had up to 12 children, the youngest being three with one staff member until 8:30 am when the second staff member arrived. According to staff statements, two staff stated the facility was without a sanitizer solution for a period of time and the preschool classroom floors were not being mopped daily. There is no evidence to substantiate regarding lack of positive behavior supports, lack of supervision, accident/injury procedures, and not using age-appropriate music because interviewed staff denied the allegations. A concern noted was upon reviewing attendance records for the day of the visit, LS #1 observed Staff #2 was present but had not signed into the attendance record, and Child #2 had only a check mark documented on the attendance record rather than a recorded arrival time.
Nov 18, 2025OCCL Standards Complaint: SubstantiatedOn October 28, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding classroom capacity and the administrator not being respectful. An investigation was conducted on October 29, 2025. The complaint is substantiated for classroom capacity and the administrator not being respectful by using threatening tones and actions when speaking and interacting with others at the center. According to attendance records provided to LS #1, Group 1 s classroom has up to twelve children present at one time, but the classroom only measures for eleven children. During interviews with staff, multiple staff members reported Administrator #1 has engaged in behavior and communication that staff members have identified as inappropriate and disrespectful.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
