The short answer
Record last released by Delaware Office of Child Care Licensing on Nov 1, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Oct 7, 2026 | OCCL Standards Complaint: Substantiated | OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. Between May 29, 2026, and June 11, 2026, the Office of Child Care Licensing (OCCL) received four complaints regarding lack of positive behavior supports. An investigation began on May 26, 2026, when the facility self-reported the incident to OCCL, and concluded on September 29, 2026. According to video footage reviewed, staff interviews, and documentation provided, this complaint is substantiated for lack of positive behavior supports. On May 26, 2026, a staff member pulled a child towards them by the child s shirt and the child s face struck the staff member s knee, causing bruising. The center terminated the staff member s employment the following day. |
| Jul 15, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On June 25, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding inappropriate behavior support. An investigation began on June 25, 2026, and concluded on July 6, 2026. Based on video footage reviewed and staff interviews, there is no evidence to substantiate this complaint for inappropriate behavior supports. Five of six staff interviewed stated they are not aware of an incident where a staff member grabbed a child by their arm too hard and they do not have any concerns about any staff member at the facility. One staff member stated they had concerns about another staff member s interaction with a child a few weeks ago but stated they did not think the staff member grabbed the child by their arm too tightly and stated the child dropped to the floor while the teacher was holding their arm to move them to another classroom. Upon review of video footage, there were no instances observed on the date and time of the allegation where a teacher grabbed a child at all and no instances where a child was upset and crying. |

| Apr 29, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. On February 3, 2026, and February 5, 2026, the Office of Child Care Licensing (OCCL) received two complaints regarding lack of supervision. An investigation was conducted on April 8, 2026. Based on information received from outside agencies, staff interviews, and licensing specialist knowledge of the physical space from previous visits, there is no evidence to substantiate this complaint. All staff interviewed stated no staff are left alone one-on-one with a child and it was noted the child s classroom s bathroom door is always left open as there is a separate stall for the toilet. |
| Apr 15, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On March 28, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to provide an accident/injury report to a parent, failing to follow their policy for handling parent complaints, and failing to document concerns regarding daily child health observations in the child s file. An investigation was conducted on April 2, 2025. There is no evidence to substantiate this complaint for failing to provide an accident/injury report to a parent. According to staff interviews, Child #1 was not injured at the center, so the center did not have to write an accident or injury report for Child #1. There is no evidence to substantiate this complaint for failing to follow their policy for handling parent complaints. Based on staff interviews, Administrator #1 followed up with the parent regarding their concerns and there was nothing seen on the video footage suggesting an injury happened at the center. There is no evidence to substantiate this complaint for failing to document concerns regarding daily child health observations in the child s file. Based on staff interviews, the concerns of prior injuries were documented on the daily attendance record and was made available to the parent. The center declined Parent #1 s request for a copy of the entire attendance record with other children s information on it. |
| Mar 17, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | On March 4, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding lack of respectful communication by staff. An investigation started on March 4, 2025, and concluded on March 14, 2025. There is no evidence to substantiate this complaint for lack of respectful communication based on staff interviews. All staff interviewed stated no staff, including Administrator #1, was rude or passive regarding the incident and all communication was respectful. |
| Mar 11, 2025 | OCCL Standards Complaint: Substantiated | On February 20, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding failure to conduct fire drills as required and providing false information to OCCL. An investigation was conducted on February 26, 2025. This complaint is substantiated for failure to document fire drills as required based on documentation provided. Although most staff stated fire drills are conducted each month and there were documented fire drills for March 2024 through December 2024, the January 2025 fire drill was not completely documented at the time it was conducted. There is no evidence to substantiate this complaint for providing false information to OCCL as, according to staff interviews, fire drills are conducted each month. |
| May 13, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On April 18, 2024, the Office of Child Care Licensing (OCCL) received two complaints regarding lack of supervision, failing to give medication to a child as required, concerns regarding maintenance of the playground, and concerns of mold inside a classroom. An investigation was conducted on April 23, 2024. There is no evidence to substantiate this complaint for lack of supervision based on staff interviews and observations made during the visit. There is no evidence to substantiate this complaint for failing to give medication to a child as required based on staff interviews. There is no evidence to substantiate concerns regarding maintenance of the playground based on staff interviews and observations made at the time of the visit. There is no evidence to substantiate concerns of mold inside a classroom based on staff interviewed and documentation received. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
