What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

DateWhatDetail
Sep 17, 2026OCCL Standards Complaint: SubstantiatedOn August 21, 2026, the Office of Child Care Licensing received a complaint regarding a lack of positive behavior supports. An investigation was conducted on August 25, 2026. This complaint is substantiated for a lack of positive behavior supports. Staff #1 and Administrator #1 stated Child #1 was hit in the back with an open hand by Staff #1 who is Child #1 s parent.
Jul 29, 2024IA Investigation: UnsubstantiatedDelaware gives no summary.
Jul 24, 2024OCCL Standards Complaint: No Evidence to Substantiate with ConcernOn February 28, 2024, a complaint was called in Child Abuse and Neglect Report Line and accepted for investigation by the Institutional Abuse Unit (IA). It was also sent to the Office of Child Care Licensing (OCCL) and accepted for investigation regarding possible improper discipline. An investigation was conducted on March 20, 2024. On May 16, 2024, Institutional Abuse Investigator stated after a Multi-Disciplinary Team meeting, the case will be closed as unsubstantiated with concern for unexplained injuries. There is no evidence for OCCL to substantiate this complaint because the staff who were with the child on February 26, 2024, denied observing any bruises on the child and denied doing anything to the child that would cause the bruises. During the investigation, several concerns were noted. There were insufficient staff because during the visit, there were not enough staff to provide bathroom breaks or answer the door. Lack of supervision occurred because two children, age three, were alone in a classroom when the staff walked out of the room to show where Ms. Bowden was located. Additionally, according to attendance records, Staff #3 was present in the Monkey infant classroom from 8:00 am until 4:37 pm on February 26, 2024, and also signed into the Elephant classroom on February 26, 2024, at 6:00 am and not signed out. No other staff were signed in on the Elephant attendance record, but eight children were signed in as present. Staff and child attendance records could not be provided for one classroom from February 26, 2024. In the one-year-old classroom, plastic bags were observed in low cubbies and a space heater was observed. Several cords were in the infants reach in the infant classroom. Safe sleep practices were not being followed because an infant was observed asleep in a pack-n-play with a blanket. Staff and child hands were not washed after a diaper change. Infant feeding procedures were not followed because an infant was observed drinking a bott le while in boppy. Lesson plans were not current because on March 20, 2024, the posted lesson plan was dated February 12, 2024. The posted daily schedule was not being followed because children were observed napping at 10:45 am but the schedule stated nap is from 12:30 pm to 2:30 pm.
Jul 3, 2024OCCL Standards Complaint: No Evidence to SubstantiateOn May 24, 2024, the Office of Child Care Licensing (OCCL) received a complaint regarding improper staff-to-child ratio, not having enough staff to operate, and being under the influence of alcohol if the phone call took place during the hours of operation. An investigation was conducted on June 6, 2024. There is no evidence to substantiate this complaint because the center was following proper staff-to-child ratio, had enough staff to operate during the visit, and the owner stated the phone call took place after child care hours but does not recall informing the parent she has been drinking.

  • Providers aren’t compared: Delaware lists only the visits that found violations, so a count can’t tell a clean record from one never visited.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
