What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Aug 17, 2026OCCL Standards Complaint: No Evidence to SubstantiateOn July 14, 2026, the Office of Child Care Licensing received a complaint alleging not following the complaint policy, mixing of ages with 10 or more children present, and meals and snacks. An investigation was conducted on July 16, 2016. There is no evidence to substantiate the complaint of not following the complaint policy because the provider tried to meet with Parent #1, who would not accept the date due to a scheduling conflict for the additional non-parent/guardian Parent #1 wanted at the meeting. When the meeting was held, Parent #1 did not address all the concerns and did not explain why. There is no evidence to substantiate the allegation of mixing age groups because the policy in the handbook was followed by transitioning two times a year, and attendance reflected that the age groups are separated upon opening. There is no evidence to substantiate the allegation that meals and snacks did not have the proper components or amount because the menu states the amount for each age group, and staff stated that there is always plenty of food for the children if they need more.
Jun 26, 2025OCCL Standards Complaint: No Evidence to Substantiate with ConcernOn May 14, 2025, and May 15, 2025, the Office of Child Care Licensing (OCCL) received two complaints regarding not meeting the needs of the children, not providing truthful information, not providing an infant daily log, confidentiality, not allowing breastfeeding, and not completing incident reports. An investigation was conducted on May 21, 2025. There is no evidence to substantiate this complaint because staff members interviewed stated they conduct diaper checks hourly, change a child if they are wet or dirty, and their daily logs match when they change diapers. Administrator #1 stated breastfeeding or pumping is welcome at the center, but a parent, who is also a staff member, needs to provide a breastfeeding schedule to ensure the classroom complies with the staff-to-child ratio. Additionally, Administrator #1 stated she or her staff did not breach confidentiality, but if a parent has a concern, she will address the concern with all her staff. Administrator #1 and staff members denied the allegation of not providing truthful information to parents and stated children enrolled in the center receive a daily update via the Tadpole app. While there is no evidence to substantiate not completing incident reports, a concern noted was the incident report from the Tadpole app does not reflect when the parent was informed of a serious injury, when a call was made to the parent, and did not have parent signatures.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
