What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.
What did inspectors find?
Delaware’s list shows only the visits that found violations, so a zero can’t be read either way: this provider may have had visits that found none, or no visit at all. No count is shown.

| Nov 17, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing (OCCL) received complaints on October 29, 2025, and October 30, 2025, regarding failure to provide an injury report and notification to a parent, failing to notify OCCL of an injury requiring medical care, and keeping an infant in confining equipment for more than 30 minutes while awake. A self-report was received by OCCL from the facility on October 29, 2025, regarding these concerns. An investigation was conducted on November 7, 2025. There is no evidence to substantiate this complaint. Based on information received through a self-report and staff interviews, the timeframe in which the children were in their cribs confirms the children were both awake and asleep during this time period. Based on staff interviews, administration statements, and documentation, there is no evidence to substantiate this complaint for failing to provide notification and a written injury report to the parent as the administrators notified Person #1 who is authorized by Parent #1 to receive injury information. OCCL was not notified due to the child care center being unaware Child #2 received medical care. |
| Jan 28, 2025 | OCCL Standards Complaint: Substantiated | OCCL s investigation of regulatory non-compliance began after clearance from other agencies was received. The Office of Child Care Licensing received a complaint on December 18, 2024, regarding allegations of improper discipline. An investigation was conducted on December 19, 2024. Based on interviews and administration statements, this complaint is substantiated for improper discipline. An unannounced visit was conducted on April 22, 2024. There is no evidence to substantiate this complaint regarding lack of supervision. Interviews with the administrator and staff showed staff members are able to supervise the classroom and assist with bathroom needs and are given assistance from Staff #3 or administration when requested. The staff members explained where they position themselves to change diapers while observing children at play. |
| Nov 20, 2023 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing (OCCL) received a complaint on October 26, 2023, regarding concerns about lack of supervision, ratios, and not following parent handbook policies. An investigation was conducted on October 30, 2023. There is no evidence to substantiate this complaint for concerns about lack of supervision based on video footage observed and interviews conducted. Staff members present on the playground responded to Child #1 s behavior and intervened. There is no evidence to substantiate this complaint for improper staff-to-child ratios based on interviews conducted and review of attendance records. All classrooms were within proper staff-to-child ratio during the visit. There is no evidence to substantiate this complaint for not following parent handbook policies based on interviews conducted and documentation viewed for Child #1. The facility s policy for termination states a child enrollment may be terminated if they create a risk of harm to other children or staff. Child #1 was observed to be physically aggressive with other children, and multiple previous incidents were documented. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
