What these words mean4
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

Oct 23, 2026OCCL Standards Complaint: No Evidence to Substantiate with Concern: On September 22, 2026, LS #1 received a self-report from Administrator #1 regarding a confrontation between parents that had occurred at the center on September 21, 2026. On September 23, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding allegations of a lack of building security procedures, failing to follow procedures for handling parents’ complaints, disrespectful communication by staff, incorrect staff-to-child ratios, and inappropriate behavior support. An investigation began on September 23, 2026, and ended on September 29, 2026. There is no evidence to substantiate this complaint for lack of security procedures because Administrator #1 and both staff interviewed stated children are not allowed to open doors for parents. There is no evidence to substantiate this complaint for failing to follow procedures for handling parents’ complaints, disrespectful communication by staff, or inappropriate behavior supports. Administrator #1 and both staff interviewed stated Staff #1 was never disrespectful to Child #1 or any other child, and Staff #1 never used inappropriate behavior supports with Child #1 or any other child. All staff interviewed denied Parent #1 complaining about situations that were not addressed by staff. There is no evidence to substantiate this complaint for incorrect staff-to-child ratios as attendance records reviewed documented all classrooms to be in correct staff-to-child ratio. A concern noted at the visit was Staff #2 stepped out of the classroom to take Child #1 to Parent #1 during a confrontation between parents in the hallway, leaving the classroom unsupervised.
Sep 10, 2026OCCL Standards Complaint: No Evidence to Substantiate with ConcernOn August 24, 2026, the Office of Child Care Licensing received a complaint regarding allegations of a lack of supervision and staff using a cellphone while responsible for caring for children. An investigation was conducted on August 28, 2026. After speaking with the administrator and the staff who were on the trip, there is no evidence to substantiate this complaint for lack of supervision or staff using a cellphone while responsible for caring for children. Administrator #1 stated they used the cell phone to call the center to have Staff #1 come and assist. Staff #1 stated they arrived at the park to assist, the children had access to water, and they checked in on Child #1 who was resting. Administrator #1 said Child #1 was resting, not actually sleeping and refused to move into the shade when asked to do so and stated Child #1 had access to water. An additional concern noted was inaccurate documentation of staff attendance. Staff #1 was documented as having been with the group the entire time, although they had remained at the center and only came to the park after being requested to do so by Administrator #1.
May 28, 2025OCCL Standards Complaint: SubstantiatedOn May 9, 2025, the Office of Child Care Licensing (OCCL) received a complaint regarding incorrect staff-to-child ratios. An investigation was conducted on May 14, 2025. Based on interviews and a review of attendance records, this complaint is substantiated for incorrect staff-to-child ratios. At the time of the investigation visit a classroom was observed to not be in correct staff-to-child ratio. In addition, a review of attendance records showed the two-year-old classroom having an incorrect staff-to-child ratio on May 9, 2025. Additional concerns were noted during this investigation. LS #1 observed an infant sleeping in a swing while wrapped in a fleece blanket. A staff member was working at the center without having been determine eligible or provisionally eligible by the Criminal History Unit, and both staff and child attendance had not been documented accurately.
Aug 7, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on July 22, 2024, regarding concerns of incorrect staff-to-child ratio and failing to complete an incident report and notify parents of an incident in the required time frame. An investigation was conducted on July 26, 2024. There is no evidence to substantiate this complaint for concerns of incorrect staff-to-child ratio based on attendance and video footage observed. The attendance records and video footage showed there were eight school-age children present with one qualified staff member. There is no evidence to substantiate this complaint for failing to complete an incident report and notify parents in the required time frame based on interviews conducted and documentation collected. Staff interviewed stated parents were notified of Child #1 s behaviors and there was a written statement regarding Child #1 behaviors.
Mar 15, 2024OCCL Standards Complaint: No Evidence to SubstantiateThe Office of Child Care Licensing (OCCL) received a complaint on February 16, 2024, regarding concerns of improper discipline and not following the facility s suspension policy. An investigation was conducted on February 21, 2024. There is no evidence to substantiate this complaint for improper discipline based on staff interviews conducted. Staff members stated files fell on Child #1 s head when she was crawling on the floor in the office. There is no evidence to substantiate this complaint for a violation of the center s suspension policy based on interviews conducted and documentation. Staff stated Child #1 was suspended from the facility, and they followed the facility s suspension policy and incident reports viewed.

What the record doesn’t say

  • Delaware doesn’t grade violations by severity, so read what was cited.
  • Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
  • Delaware publishes five years of reviews and complaints; this page shows the last three.
  • Delaware doesn’t publish staffing levels or owners for individual providers.
  • Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
