The short answer
Record last released by Delaware Office of Child Care Licensing on Nov 4, 2026.
Each figure’s source sits beside it, in its section below.
What these words mean4
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

| Date | What | Detail |
|---|---|---|
| Jun 3, 2026 | OCCL Standards Complaint: Substantiated | On May 18, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding infant feeding requirements, infant sleep safe, and infant daily log requirements. An investigation was completed on May 19, 2026. Based on interviews and documentation reviewed this complaint is substantiated for failing to follow infant feeding requirements because a staff member stated they sometimes feed an infant while the infant is on the floor. A picture in the Bright Wheel app documented an infant drinking a bottle while on the floor on May 6, 2026. Based on interviews and documentation reviewed, there is no evidence to substantiate this complaint for failing to follow infant sleep safe practices. Based on interviews and documentation reviewed, there is no evidence to substantiate this complaint for failing to complete infant daily log requirements. |
| May 13, 2026 | OCCL Standards Complaint: No Evidence to Substantiate | On April 20, 2026, the Office of Child Care Licensing (OCCL) received a complaint regarding failing to follow child suspension/expulsion policy, failing to follow policy for handling complaints, and not following staff-to-child ratio. An investigation began on April 24, 2026, and concluded on April 29, 2026. Based on video footage viewed and documentation collected, there is no evidence to substantiate this complaint for failing to follow child suspension/expulsion policy. The Lighthouse Early Childhood Center (LECC) termination policy states LECC may choose to terminate for any cause agreed upon by the Administration. In this case, termination of care was due to unsafe or hostile behavior toward staff by parent/guardian. Based on documentation reviewed, there is no evidence to substantiate this complaint for failing to follow policy for handling complaints. The center had been working with the family to support Child #1 s behaviors. Based on documentation collected and observation at the visit, there is no evidence to substantiate this complaint for not following staff-to-child ratio. Observations at the visit and attendance records reviewed show the classrooms to be in correct staff-to-child ratio. |
| Jul 25, 2025 | OCCL Standards Complaint: No Evidence to Substantiate | The Office of Child Care Licensing (OCCL) received a complaint on July 17, 2025, regarding concerns of lack of supervision. An investigation was conducted on July 21, 2025. There is no evidence to substantiate this complaint for concerns of lack of supervision based on the staff interview, staff statements reviewed, and video footage reviewed. Staff interviewed and staff statements consistently noted Child #1 lost his balance at the top of the slide and fell down the slide with staff present and attending to children. LS #1 observed video footage of the playground with two staff members engaging in play with nine children. |
| Nov 14, 2024 | OCCL Standards Complaint: Substantiated | The Office of Child Care Licensing (OCCL) received a complaint on October 25, 2024, regarding concerns of failure to follow illness exclusions. A telephone investigation was conducted on October 28 and 29, 2024. This complaint is substantiated because a screenshot of Staff #1 s conversation with Administrator #1 shows Staff #1 state they had a fever along with Child #1, and Administrator #1 s response was that they would tell Staff #1 to stay home, but they were already short a staff member for the day. Staff #1 then stated they and Child #1 would take medication and come in. |
| May 20, 2024 | OCCL Standards Complaint: No Evidence to Substantiate | On May 14, 2024, the Office of Child Care Licensing received a complaint regarding not following the center s breastfeeding policy by not allowing a parent to breastfeed at the center. There is no evidence to substantiate this complaint because the center administrator and Staff #1 both denied not allowing a parent to breastfeed at the center. The center administrator also provided LS Flowers with a copy of the center s breastfeeding policy and showed LS Flowers the private breastfeeding lounge. |

What the record doesn’t say
- Delaware doesn’t grade violations by severity, so read what was cited.
- Delaware’s open data lists a visit only when a violation was cited there, so visits that found nothing aren’t shown.
- Delaware publishes five years of reviews and complaints; this page shows the last three.
- Delaware doesn’t publish staffing levels or owners for individual providers.
- Delaware lists only the visits that found violations, so a provider with none listed may not have been visited at all: no count of violations is used to headline, filter or compare its providers.
