What Wisconsin’s list coversSettings for older adults

Community-based residential facilities and adult family homes licensed for older adults or people with dementia, and residential care apartment complexes. Those licensed only for other adults, such as people with developmental disabilities, are left out on purpose. Adult family homes for one or two people are certified separately and aren’t here.

What these words mean7
Not read
The file this site reads doesn’t carry what inspectors found, so nothing is counted here. It doesn’t mean the place has a clean record: the state’s own site may have its reports. Source: How this site reads each source.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Memory care
Care for people living with dementia. Some states license a separate dementia or Alzheimer’s unit; others ask a facility to say what dementia care it gives. This record uses the state’s own field. Source: Each state’s list.
Accepts Medicaid
The state’s own list says the place takes part in a Medicaid program (a waiver, a contract or a program like Illinois’s supportive living) that can pay for care for people who qualify. Source: Each state’s list.
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

What did inspectors find?

Wisconsin’s assisted living directories carry no inspection results, so this page can’t say what inspectors found. The Division of Quality Assurance says of its Provider Search: “Many providers have a Survey History section. This shows surveys that DQA did in the last three years. … Surveys can tell you how many violations a provider has had.”

  • Wisconsin’s assisted living directories carry no inspection results, so this page can’t say what inspectors found. This shows surveys that DQA did in the last three years. … Surveys can tell you how many violations a provider has had.”
  • Wisconsin’s directories list a community-based residential facility or an adult family home here only where its licence serves the advanced aged or people with dementia.
