The short answer
Record last released by Texas Health and Human Services Commission, Long-Term Care Regulation on Nov 4, 2026.
- 1 at the state’s most serious level, in three years (needs attention)
- 6 residents, its licensed capacity
Each figure’s source sits beside it, in its section below.
What these words mean5
- Inspection (survey)
- A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
- Deficiency, citation, violation
- What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
- The state’s most serious level
- Each state grades findings on its own scale and in its own words, so the record counts a state’s findings at that state’s top level. Two states’ top levels aren’t the same thing, so the counts aren’t compared across states. Source: How this site reads each state’s scale.
- Licence status
- Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
- Capacity
- The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.
What did inspectors find?
HHSC’s facility visit history lists 6 visits to this facility that ended from May 13, 2024 to Jun 23, 2026, and 11 state violations cited at them: 1 at level 3 or 4 of HHSC’s four, 1 of them at level 4.
Each violation carries the scope and severity letter HHSC’s file gives it, from A, the least serious, to L, the most. HHSC’s Assisted Living Facility Data (revised March 2026) counts the letters in four levels: level 1 is A, B or C; level 2 D, E or F; level 3 G, H or I; and level 4, J, K or L, the level whose tags it counts as immediate threats. Within a level, the first letter is a violation found in isolation, the second a pattern, the third widespread, as on the scope and severity grid CMS uses for nursing homes, whose letters HHSC’s levels follow. A violation HHSC’s file gives 0 for, rather than a letter, has no scope and severity in the file.
J: level 4 of HHSC’s four (J, K or L), the level HHSC counts as an immediate threat; its scope isolated: Rights-Res Bill of Rights, 26 TAC §553.267(a)(3)(E)(ii), Mar 19, 2026

When those violations happened
11 violations in 3 months of the last 45. Last cited Mar 2026. Most serious: level 4 of HHSC’s four (J, K or L), the level HHSC counts as an immediate threat; its scope isolated (J). The last 4 years, to Nov 5, 2026.

What the record doesn’t say
- HHSC’s facility visit history gives each visit’s days and kind, and each violation’s rule tag and scope and severity letter; it doesn’t give what inspectors wrote, which is in each visit’s own record, which HHSC’s Long-term Care Regulation sends on request, by the survey’s exit date. It names no visit of a facility under another ID.
- Copied from the Texas Health and Human Services Commission’s website, with its address and the date shown in the source line. Neither this information as presented here, nor this site, is endorsed by the State of Texas or any state agency.
- Texas’s directory lists only facilities with an active licence; its Medicaid columns are blank.
