What Illinois’s list coversFrom two lists

Assisted living and shared housing establishments and sheltered care facilities licensed by the Department of Public Health (IDPH), from its list as of Oct 15, 2026, and supportive living facilities certified by the Department of Healthcare and Family Services (HFS), not those for adults 22 to 64 with physical disabilities. Most of IDPH’s establishments show a licence expiration date that has passed.

What these words mean7
Not read
The file this site reads doesn’t carry what inspectors found, so nothing is counted here. It doesn’t mean the place has a clean record: the state’s own site may have its reports. Source: How this site reads each source.
Inspection (survey)
A visit by state inspectors, often unannounced, who check a place against the rules it must meet. For nursing homes and other places Medicare pays, CMS calls these visits surveys: a standard survey of a nursing home comes at least every 15 months, and a complaint survey whenever a complaint calls for one. Source: CMS; 42 CFR 488.308.
Deficiency, citation, violation
What inspectors write up when a place didn’t meet a rule: the rule, what they saw, and when. CMS calls it a deficiency; states may say citation or violation. It is the inspectors’ finding, not a court’s, and the place is usually asked for a plan to correct it. Source: CMS, statement of deficiencies (Form CMS-2567); each state’s licensing agency.
Certified
A government program has approved the place to take part in it, after checking it meets the program’s rules: Medicare or Medicaid, or a state program such as Illinois’s Supportive Living Program. It isn’t the same as a state licence. Source: CMS; each state’s certifying agency.
Supportive living facility
Illinois’s Medicaid alternative to a nursing home: apartments with personal care and other services, for older adults and people with disabilities. The Department of Healthcare and Family Services certifies each site; it isn’t licensed as an assisted living establishment. Source: Illinois Department of Healthcare and Family Services (hfs.illinois.gov).
Licence status
Whether the state’s licence lets the place operate now, in the state’s own words: licensed, on probation (allowed to keep operating on conditions, after problems), provisional, pending or expired. Source: Each state’s licensing agency.
Capacity
The most people the licence or certification lets the place care for at once: residents, children, beds or apartments, as the state counts them. It isn’t how many are there now. Source: Each state’s licensing agency.

What did inspectors find?

Illinois doesn’t publish inspection results for assisted living or shared housing establishments online, and IDPH’s list carries none, so this page can’t say what inspectors found. Its rules say: “Individuals may request information from the Department concerning an establishment by submitting a written request.”

  • IDPH’s Facility ID: 6021428.
  • Licensee ID in IDPH’s list: 3001021.

What the record doesn’t say

  • Illinois doesn’t publish inspection results for assisted living or shared housing establishments online, and IDPH’s list carries none, so this page can’t say what inspectors found. Its rules say: “Individuals may request information from the Department concerning an establishment by submitting a written request.”
  • IDPH’s list types each as an assisted living or shared housing establishment, one type for both, so it doesn’t say which this is. A shared housing establishment is a residence for 16 people or fewer, at least 80% of them 55 or older.
  • IDPH’s list is a file its Freedom of Information officer sent on Oct 20, 2026, saved by a person and not read automatically: it is as current as its title’s day, which the source line gives.
  • IDPH’s licensing portal has each facility’s current licence: search it by the facility’s name.
